Clinical Research On The Relationship Between Serum Uric Acid And Atrial Fibrillation In Patients With Rheumatic Valvular Heart Disease

objective:To investigate the relationship between serum uric acid level and atrial fibrillation in patients with rheumatic valvular heart diseease.Methods:Retrospective analysis of 334 hospitalized rheumatic valvular heart diseease patients who were divided into AF group(n=240)and SRgroup(n=94).It was compared between the two groups about age,gender,Ur,Cr,UA,albumin,globulin,TG,TC,LDL-C,HDL-C,c-reactive protein,anti-O,rheuma-toid factor,fibrinogen,white blood cell count,LAD,LVEDD,LVPWT,IVST,LVEF.The patients were divided into hyperuricemia group(n=151)and without hyperuricemia group(n=184)according to serum uric acid criteria.The incidence of atrial fibrillation were compared between the two groups.Further,the patients were divided into four quartile groups according to UA level.The incidence of atrial fibrillation were compared among the four groups.Results:Compared with SR group,in AF group,UA,Ur,Cr,LAD presented obviously increased(t=-9.474,-3.094,-7.684,-3.274;P=0.000,0.020,0.000,0.001),whileHDL-C,TC,albumin,LVEF,female ratio decreased(t=5.831,2.571,3.261,3.004,X2=17.151;P=0.000,0.011,0.001,0.030,0.000).With adjusted UA,Cr,Ur,LAD,female ratio,albumin,HDL-C,TC and LVEF,the Logistic regression analysis showed that the UA(OR:1.009;95%CI:1.005-1.012;P=0.000),Cr(OR:1.036;95%CI:1.006-1.067;P=0.019),LAD(OR:1.040;95%CI:1.006-1.075;P=0.019),LVEF(OR:0.952;95%CI:0.909-0.998;P=0.040)were independently related to AF.Significant difference of the incidence of AF was found between hyperuricemia group and without hyperuricemia group(84%vs5 7%;P=0.000).And the result is same among the four quartile groups(53%,63%,80%,96.4%;P=0.000).Conclusion:The UA in patients with rheumatic valvular heart diseease has a linear positive correlation with the incidence of AF,the higher serum uric acid level,the higher the incidence of AF.The AF occurrence was independently related to the elevated UA,Cr,LAD and LVEF.
