Objective:Acute hypertension with multi-organ damage(hypertension-MOD)is a new definition to describe hypertensive emergencies,which is beneficial to identify these true hypertensive emergencies.Hypertension-MOD has a much worse prognosis than the usual forms of hypertension.Early recognition and management of hypertension-MOD are fundamental to any improvement in prognosis.This study summarizes and analyzes the clinical characteristic as well as treatment experience of 20 cases of hypertension-MOD so as to prevent hypertension target-organ damage and improve patients' prognosis.Method: Collect 20 cases of hypertension-MOD in the 5th department of cardiology of the Second Affiliated Hospital of Dalian Medical University from July 2014 to December 2016.Retrospective analysis of patients' general information,medication history,etiology,precipitating factors,risk factors,treatment and prognosis.Result: 1 ? The general characteristics: Patients with hypertension-MOD were predominantly young men(85%).The general symptoms of the patients were diverse,15 cases complained of headache and dizziness as the main symptom(80%),1 case was no clinical symptom.2?Precipitating factors: Infection(35%),mood swing(25%)and irregular medication(20%)were the main precipitating factors of patients with hypertension-MOD.3 ? Risk factors: The risk factors of hypertension-MOD were overweight,high fat,high salt diet and smoking,etc;19 cases were overweight patients(95%),16 cases were high fat food takers(80%),15 cases were high salt takers(75%),14 cases were smoking patients(70%),and 13 cases were patients with early-onset hypertension family history(65%).4?Etiology: 9 cases among the 20 cases of hypertension-MOD patients were diagnosed as primary hypertension(45%).There were 6 cases of hypertension diagnosed for the first time(30%).5?The damage of the target organs:Multi organ damage mainly involved the damage of heart,kidney and brain: 7 cases with heart,brain and kidney damage(35%);2 cases with heart as well as brain damage and hypertensive retinopathy(10%);And 3 cases with acute injury in heart,brain,kidney and hypertensive retinopathy(15%).6?Treatment and Prognosis: In acute phase,applied positive anti-hypertensive therapy with intravenous administration combined oral administration.If patients' blood pressure reached to the target within 1-2 weeks and was maintained stable for a long time,the partial functions of damaged target organs can recover with better prognosis.Conclusion: Patients with hypertension-MOD were mostly young male patients.Among hypertension-MOD patients,the risk factors mainly are overweight,high fat,high salt diet and smoking,etc;infection,mood swing and irregular medication are the main precipitating factors;Etiology mainly is primary hypertension;Multi organ damage mainly involved the damage of heart,kidney and brain.After anti-hypertensive therapy,if patients' blood pressure reached to the target within 1-2 weeks and is maintained stable for a long time,the partial functions of damaged target organs can recover with better prognosis.
