Effect Of Huanglian Wendan Decoction On LCAT And EL In Patients With Type 2 Diabetes Complicated With Lipid Metabolism Disorder

Objective:Through the application of huanglianwendan Decoction in the treatment of syndr ome of phlegm heat syndrome of type 2 diabetes mellitus and lipid metabolism disorders,observation of Huanglian Wendan Decoction on body mass index,th e patients blood glucose(fasting blood glucose,2h postprandial blood glucose,glycosylated hemoglobin,fasting insulin,blood lipid(three)glycerol esters,total cholesterol,low density lipoprotein,high density lipoprotein,apolipoprotein A1 and apolipoprotein B,lecithin cholesterol acyl transferase and endothelial lipas e),TCM syndrome score and symptom improvement,intervention effect evaluati on of huanglianwendan Decoction in the treatment of disorders of lipid metaboli sm in type 2 diabetic patients.Methods:60 patients diagnosed as type 2 diabetes combined with lipid metabolism abnor mality were randomly divided into 30 cases in the control group and 30 cases i n the observation group according to the random number table.Two groups of patients were based on the conventional antidiabetic drugs,the control group wa s treated with statin therapy,the observation group(Atorvastatin Calcium Tablet s)for Atorvastatin Calcium Tablets huanglianwendan decoction.A 3 month obse rvation was carried out.The body mass index(BMI),blood glucose and lipid r elated indexes,serum LACT and EL activity level,TCM syndrome score and s ymptom improvement of two groups before and after treatment were observed.To explore the clinical intervention effect of Huanglian Wendan Decoction on p atients with type 2 diabetes with lipid metabolism disorder.Results:Finally,60 patients with type 2 diabetes mellitus complicated with lipid metabol ism disturbance and phlegm heat syndrome were enrolled.The number of patien ts in the observation group and the control group accounted for half.Effective follow-up was 59 cases,and only 1 cases in the control group.Before treatmen t,there was no statistically significant difference in the general situation betwee n the two groups(gender,age,course of disease,TCM syndrome score,blood sugar level,etc.),and the difference was not statistically significant.(1)efficacy of TCM syndromes: after treatment,the symptoms and signs in the observation group were improved to varying degrees,and the total score of TCM syndrom e was significantly lower than that in the control group(P<0.01).The blood gl ucose: after treatment,two groups of patients with fasting blood glucose(FPG),2h postprandial blood glucose(2h PG),glycosylated hemoglobin(Hb A1c)and i nsulin resistance index(HOMA-IR)were significantly decreased,insulin sensitivi ty index(ISI)increased significantly,compared with before treatment was statist ically significant(P<0.05,P<0.01);comparison between groups,the observation group improved more significantly,with statistical significance(P<0.01).The blo od lipid: after the treatment,the observation group of patients with total cholest erol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)levels decreased significantly,high density lipoprotein cholesterol(HDL-C)and apolipo protein A1(Apo A1)levels increased significantly,compared with before treatme nt was statistically significant(P<0.01).The levels of TC,LDL-C and apolipopr otein B(Apo B)in the control group decreased,while the levels of HDL-C and Apo A1 increased.There was a statistically significant difference compared with that before treatment(P<0.01),but the TG level in the control group was not significantly improved compared with that before treatment(P>0.05).Compared with the control group,the levels of TG,TC,HDL-C,LDL-C and Apo A1 in t he observation group improved more significantly than those in the control grou p(P<0.01),but there was no significant difference in Apo B level between the two groups(P>0.05).LCAT,EL: after treatment,patients in the observation gro up the level of LCAT activity was significantly increased compared with that b efore the treatment,EL activity significantly decreased,there was significant diff erence(P<0.01);the control group LCAT,EL activity levels improved significan tly,compared with before treatment there was no significant difference(P>0.05);LCAT and EL groups.There was statistical significance(P<0.01).Safety index:the blood and urine routine,electrocardiogram,liver and kidney function and c hest radiograph in the two groups were all in the normal range during the treat ment.Until the end of treatment,there were no obvious adverse reactions in th e two groups.Conclusion:The clinical research results show that,the application of Western medicine combined with Huanglian Wendan Decoction in the treatment of type 2 diabetes mellitus and lipid metabolism in patients with phlegm heat syndrome is better than pure western medicine treatment,not only can improve the clinical symptoms and signs of patients,reduce blood glucose,regulating blood lipid,the level of activity can improve the serum LCAT,EL,without obvious side effect.This study provides a scientific basis for the application of Huanglian Wendan Decoction in the treatment of type 2 diabetes mellitus with lipid metabolism disturbance and phlegm heat mutual syndrome,which lays a foundation for further research of Huanglian Wendan Decoction.
