Objective:Upper gastrointestinal hemorrhage is a common disease of gastroenterology.But arterial esophageal fistula is the one of the rare factors.There was one case report about arterial esophageal fistula in a small branch of aorta by esophageal carcinoma invasion from the department of gastroenterology of Nanjing first hospital.We reviewed some similar literatures as follow.Methods:The main points of diagnosis and treatment of arterial esophageal fistula are studied through case analysis and literature review.Results:This patient suffered from upper gastrointestinal bleeding suddenly.The doctor conducted two emergency endoscopy examinations but failed to clarify the bleeding point.The patient appeared upper gastrointestinal bleeding again and came to be shocked.Through the emergency surgery we definited the patient’s upper gastrointestinal bleeding cause was arterial esophageal fistula in a small branch of aorta by esophageal carcinoma invasion.He recovered after the operation.Through reading the case,esophageal foreign body is the most common cause of arterial esophageal fistula,which is elderly patients and those with atherosclerosis more likely to suffer from.Conclusion: Upper gastrointestinal bleeding with unclear reasons should be performed vascular interventional examination or emergency surgical treatment as soon as possible.The patient had a sudden onset.The gastroscopy diagnose before being hospitalized was esophageal occupation.Emergency endoscopy and routine imaging examination failed to identify the bleeding point,and the patient was delayed by subjective factors.The recognition of Chiari’s triad is the focus of diagnosis and treatment of arterial esophageal fistula.
