ObjectiveAnalyze the possible risk factors for pancreatoduodenectomy(PD)after complications,in order to effectively control these factors before and after surgery to achieve the purpose of reducing complications.MethodsFrom June 2013 to October 2017,there were 80 patients with PD due to ampullary carcinoma or pancreatic head cancer in Hepatobiliary Surgery of the First Affiliated Hospital of Zhengzhou University.Seven possible risk factors were selected: age,preoperative serum bilirubin levels,preoperative serum albumin levels,preoperative jaundice duration,surgical time,intraoperative blood loss,tumor size,and quantitative assignments.Statistics were made on the data of each case to obtain the complication rate and mortality rate during hospitalization.Univariate analysis of seven possible factors was performed using chi-square test.P<0.05 was considered statistically significant.The factors screened by univariate analysis were analyzed by logistics.P<0.05 was considered statistically significant.Results7 factors were analyzed by single factor analysis.4 independent factors(P < 0.05)were found: preoperative serum bilirubin level was more than 170umol/l,preoperative jaundice duration was more than 20 days,operation time was more than 6h,and the amount of intraoperative bleeding was more than 1000 ml.The above 4 factors were further tested by logistics analysis,and 3 related independent factors(P < 0.05)were finally obtained.The preoperative serum total bilirubin was more than 170umol/l,the duration of preoperative jaundice was more than 20 d,and the amount of intraoperative bleeding was more than 1000 ml.The time of operation failed to enter the regression model.ConclusionPerioperative management of preoperative hyperbilirubinemia,prolonged jaundice,and high intraoperative blood loss may reduce complications after pancreatoduodenectomy.
