Clinical Case Report Of Implant-supported Prosthesis In The Maxillary Anterior Dentition Defect Rehabilitation

Background:For the cases of missing natural tooth,it’s very different of missing anterior teeth against to the posterior zone.If an anterior tooth is lost,it will cause not only the pronunciation problems to the patient,also imperfect the face and self-image.Thus affect to lead personal negative emotions and lose confidence.Therefore,to restore anterior edentulous rehabilitation with prosthesis replacement is much more important.The replacement of missing tooth was taken for treatment firstly by the removable partial dentures-in early use,advancing to the popular porcelain crown/bridge restorations and till the latest implant-supported prosthesis reconstruction.To rehabilitate the anterior edentulous has being in development rapidly.So far,in view of the rehabilitation or the prognosis effect,implant prosthesis is the best solution.Dental implant supported prosthesis have been used in edentulous rehabilitation for over 50 years.The early application of implant supported prosthesis have been mainly taken to improve the problems or bad-conditions caused by the full denture due to the excessive bone loss such as alveolar bone atrophy,masticatory dysfunction,as a consequence the denture became loose.With the recent development,the implant supported prosthesis also successfully solved the problems of hard/soft tissue loss.In the past twenty years,the progress of bio-technologies and the improvement of dental imaging technologies,as well as the aesthetics restoration are more and more accepted by the public.The implant-supported anterior restoration of aesthetics are also in fast development.The ultimate aim of ideal implant supported dental reconstruction is to attempt reproducing a functional and esthetic replacement of missing teeth as the same appearance and function as the nature teeth.If patient follow doctor’s advice to care and regular maintenance on the implant-supported prosthesis,the life-time of implant restoration will be extended longer.The immediate implantation is a kind of restoration of Immediate Implant Placement.Compared with the normal delayed implantation or late,the immediate implantation reduces the risk of bone absorption after tooth extraction and is more conducive to preserving alveolar ridge and longevity of the gums,so that the implant can be placed well in a desirable position.In addition,to obtain the prosthesis for aesthetic design in more perfect anatomy contouring and vital shading to match the other natural teeth around.Therefore,after tooth extraction,to reserve the height,width and surrounding soft tissues of the alveolar ridge are very important for the anterior restorative in reconstruction.Nowadays,there are mainly guided bone regeneration,site preservation,gingiva induction surgery and other methods and solutions to assist the implant surgery of the hard and soft tissue of the implant site,in order to achieve the ideal aesthetic restoration of the anterior dental implant.In addition,immediate implantation shortens the surgery time of the whole treatment and rehabilitation,reduces the patient’s psychological pressure,enhances the postoperative satisfaction of the implant surgery.Immediate Implant Placement can be promoted for more clinical application.Objection: To rehabilitate the function and aesthetics of the maxillary anterior zone of the patient is the standard to be considered as the success of the maxillary anterior implant-supported restoration.In this study,the surgery of immediate implantation and delayed-implantation were performed with treatments on patients for rehabilitation of different teeth missing on anterior zone.To observation on the prognosis and clinical effects,so as to provide the records as a clinical guidance for the selection of implants-supported prosthesis in future treatment of maxillary anterior teeth missing replacement.Methods: The subjects of study were 10 patients(aged from 40 to 80 years)who underwent implant-supported prosthesis of maxillary anterior teeth at Jing Zheng detal clinic from January 2015 to December 2017.All patients signed to agree the informed consent form.The above patients were divided into 2 groups,5 patients in each group.The first group of those cases of implant-supported prosthesis taken with immediate implantation on maxillary anterior missing teeth,and the second group of those cases implant-supported prosthesis taken with the delayed-implantation on maxillary anterior missing teeth.After 3 to 6 months,all patients in both two groups were completed the 2-step implant surgery and with wearing the temporaries for moulding with gum shaping and fitness.In 6 to 12 months after final treatment,to check-up the osseointegration of bone tissue and also to see if performed the ideal aesthetics of prosthesis.Results: Compared the first group with the second group,the alveolar bone length and the gingival shape recovered well in the first group.6 to 12 months after completion of the permanent prosthesis to observe the therapeutic effect,the imaging results show that the first group and the second group did not find any transmission around the implant and not pathological bone absorption.No infection or irreversible pain was found after implantation treatments.The implants achieved good osseointegration and the prosthesis in ideal aesthetics.The patients were satisfied with the course of treatment and the final rehabilitation effect.Conclusion: Immediate implantation restoration in the maxillary anterior zone can achieve good osseointegration.Aesthetic prosthesis restoration can meet the aesthetic requirements of the patients.It is a feasible and reliable method to rehabilitate the missing tooth.
