| Objective To compare the effect of modified gypsum decoction on osseointegration before and after treatment of chronic periodontitis patients,and to provide some reference value for clinical diagnosis and treatment.Methods From January 2017 to December 2017,the department of stomatology,the first affiliated hospital of hunan university of traditional Chinese medicine,was diagnosed with chronic periodontitis and 60 patients with dental implant defect.Among them,29 were male,31 were female,the oldest was 80 years old,the youngest was 22 years old,and the average age was 45.75 plus or minus 2.04 years old.At least one implant was implanted in each patient’s mouth,and a total of 77 implants were implanted in the German Ankylos planting system,with 24 implants and 53 non-embedded implants.All patients had oral health education and routine periodontal treatment before operation,and 60 patients were randomly divided into treatment group 30 cases and control group 30 cases.The treatment group was treated with conventional antibiotics and decoction with a combination of conventional antibiotics and new treatment.Among them,the conventional antibiotics were stopped after 5d,and the combination of the decoction and the convalescent liquid was one course of treatment,with a total of three courses,and the interval between each course was 10 d.Treatment group and control group before and after treatment 1 month and 3months after treatment,respectively,record the clinical state of patients with periodontal indexes,bleeding index(BI),periodontal probing depth(PD),clinical attachment loss(CAL),and through the blood test serum osteocalcin(BGP)and alkaline phosphatase(ALP)activity in osteoblast,CBCT scanning observation around implant the HU,dual-energy X-ray absorptiometry(dexa)in patients with bone mineral density testing,as well as the analysis of the data using statistical methods.Results In the treatment group and the control group,the distribution of gender and age was shown in the two groups: P BBB 0.05,no statistical significance,andcomparability.The treatment group and control group comparison between groups,and compare group results showed that the treatment group and control group before and after treatment in two way 1 month,3 months after treatment the BI,PD,CAL value all have significant differences,there is statistical significance,P < 0.01;There was no significant difference in BI,PD and CAL values between the treatment group and the control group before and after treatment for 1 month after treatment and3 months after treatment,without statistically significant difference.There was no significant difference between the treatment group and the control group before the treatment of serum BGP and ALP.Serum BGP level tests,before and after treatment 1month,3 months after treatment,the treatment group compared with the contrast between the way it is significant difference,the treatment group after treatment serum BGP and ALP period was significantly higher with the control group(P < 0.05);The difference was also significant in the treatment group after 1 month and 3 months after treatment(P<0.05).Serum ALP level tests,before and after treatment 1 month,3months after treatment,the treatment group compared with the contrast between the way it is significant difference,the treatment group than the control group with time high serum BGP and ALP(P < 0.05);There was also significant difference between the treatment group before and after treatment in January and after treatment(P <0.05).CBCT testing around the implant bone density,the result shows that the treatment group after treatment around the implant bone mineral density is significantly higher than before the treatment,treatment of bone around implant after treatment group was obviously higher than that of control group,was statistically significant(P < 0.05);Compared with the control group,there was no significant difference in bone density around the implant for 3 months after treatment.The results showed that BMD was significantly different in the treatment group before and after treatment for 1 month after treatment and 3 months after treatment,with statistical significance,P < 0.05;The control group was not statistically significant before and after treatment for 1 month and 3 months after treatment.There was significant difference in BMD between the treatment group and the control group before and after treatment for 1 month and 3 months after treatment,with statistical significance,P < 0.05.Conclusion Modified gypsum decoction can be used as a clinical guidance forpatients who suffer from chronic periodontitis and dental defect.Modified gypsum decoction can significantly improve the bone mineral density around the implant and improve the survival rate of implant and the success rate of planting and repair.It has good clinical effect in treating chronic periodontitis with the addition of flavor and fresh liquid,and it is worth popularizing. |
