| ObjectiveThe study evaluated the clinical efficacy and the level of serum IL-4of traditional Chinese medicine based on the method of reinforcing the earth to strengthen the metal for treating in stable COPD patients with Qi deficiency of the lung and spleen syndrome,to provide the clinical basis and to investigate the change of serum IL-4 of the method of reinforcing the earth to strengthen the metal in stable COPD patients.MethodsAccording to the inclusion and exclusion criterion of the stud y,50 cases of stable COPD patients were admitted to the study divided int o the treatment group and control group under the principle of rand om and comparison,the control group set up a treatment plan according to the GOLD2017 guideline for the stable COPD patients,only treated wit h western medicine,the treatment group received western medicine and t he method of reinforcing the earth to strengthen the metal,25 cases in ea ch group.The course of treatment is 8 weeks,then to compare the scores of TCM syndrome,pulmonary function,CAT score,six minute walking distan ce change,and detect the level of IL-4 in peripheral blood of two groups before and after the treatment by ELISA,and make a scientific evaluation with statistical methods.Results1.Effect on clinical efficacy:The total efficiency of the treatment group was 81.6%,while the total effective rate of the control group was 63.6%,the total efficiency of treatment group was higher than control group,but the difference was not significant(P > 0.05).2.Comparison of TCM Syndrome Scale of each symptom score: before treatment,two groups of patients with various TCM syndromes,the difference was not statistically significant(P>0.05),after treatment,the rest symptom scores of the treatment group were lower than that before treatment except the symptoms of susceptibility to diseases and spontaneous sweating,there was statistical significance(P<0.05),after treatment,the symptom scores of cough phlegm,wheezing,shortness of breath and fatigue in control group were significantly decreased(P<0.05),but there was no obvious differences in anorexia,abdominal distension,diarrhea,spontaneous sweating,susceptibility to diseases symptom scores(P>0.05);the symptom score of sputum in treatment group decreased more significantly than the control group,the difference was statistically significant(P<0.05)3.Lung function assessment:Before treatment,two groups of patients with pulmonary function indexes were comparable(P>0.05).After treatment,the FVC and FEV1% pre of two groups both were higher than before,the difference was statistically significant(P<0.05),but the FEV1/FVC had no obvious change,there is no statistical significance(P>0.05).The difference between the two groups before and after treatment,the FVC of treatment group was significantly higher than that in the control group,there was statistically significant(P < 0.05),while there was no significant difference between FEV1%and FEV1/FVC before and after treatment in two groups(P>0.05).4.CAT score :Before treatment,the CAT score of two groups were comparable(P>0.05).After treatment,the CAT score in the control group and the treatment group all decreased compared with those before,the difference was statistically significant(P<0.05),compared the CAT score difference between the two groups before and after treatment,the CAT score difference in the treatment group were significantly more than control group(P<0.05).5.6 minutes walk distance(6MWT):Before treatment,the 6MWT of two groups were comparable(P>0.05).After treatment,the 6MWT in the two groups was higher than before the treatment(P<0.05),and the treatment group was more obvious higher than the control group(P<0.05).6.Serum IL-4:The level of IL-4 is not related to the grade of COPD.There is no significant differences between two group about the level of IL-4 before the treatment(P > 0.05).After treatment,the level of IL-4 in treatment group and control group were significantly decreased compared with before,the difference was statistically significant(P<0.05),but there was nosignificant difference between the two groups before and after the treatment of IL-4(P>0.05).Conclusion1.The method of reinforcing the earth to strengthen the metal combined with conventional western medicine treatment of stable COPD patients with Qi deficiency of the lung and spleen syndrome,can significantly improve the clinical symptoms,especially in sputum,anorexia,abdominal distension,the effect is more obviously.2.The method of reinforcing the earth to strengthen the metal can improve the patient’s lung function,there is a certain effect to slow the downtrend of lung function.3.The method of reinforcing the earth to strengthen the metal combined with conventional western medicine treatment is better than western medicine in improving the patient’s endurance.4.There is no sufficient evidence that serum IL-4 level is associated with the degree of airflow limitation in patients with stable COPD.5.Both of The method of reinforcing the earth to strengthen the metal combined with conventional western medicine and western medicine treatment can reduce the level of serum IL-4.The relief of the clinic symptoms and the improvement of activity endurance may associated with the reduction of serum IL-4. |
