Objective1.Through literature research and clinical investigation,exploring the risk factors and protective factors of colorectal polyps,to provide a reference for colonoscopy in high-risk population,and provide guidance for patients to improve their life style and prevent diseases.2.Based on the results of syndrome differentiation of senior physicians in our hospital,the syndrome differentiation and collation of colorectal polyposis was proposed to explore the distribution of syndromes and to provide information for the formation of consensus.Methods1.Retrieve and collect all the relevant factors mentioned in the literature in recent 10 years from CNKI,Wanfang Database,VIP Database,China Biomedical Literature Database,Pubmed,Embase Database and other databases.Then put forward personal conjectures and set up questionnaires.2.Using the case-control study design,through questionnaire survey,we collected and retrospectively analyzed the data of 2017.1.1-2018.1.31 300 eligible patients.The conclusions were drawn from the statistical analysis,which described the epidemiology characteristics and syndrome distribution of colorectal polyps,and summarized the risk factors and protective factors.Results1.The study of related factors found that Hp infections(OR:6.201,CI [1.581,24.330]),smoking(4.366,CI[1.335,14.286])and peptic ulcer(OR:4.217,CI[0.764,23.289])were strong risk factors for the development of colorectal polyps.Increasing age also increased the risk of colorectal polyps(OR: 1.092,CI[1.042,1.145]).Comparing to the poor diet cleanliness,the general diet cleanliness(OR : 0.275,CI[0.011,7.106])and great diet cleanliness(OR: 0.055,CI[0.003,1.202])were both strong protective factors for the incidence of colorectal polyps.Exercising more than half an hour every single time was also protective factors,whether 0.5-1 hour(OR:0.254,CI[0.084,0.771]),1-2 hours(OR:0.055,CI[0.015,0.206])or more than 2 hours(OR : 0.247,CI[0.024,2.564] every time.2.The study of Syndromes found that the distribution of syndromes of adenomatous polyps from high to low were: spleen insufficiency and wet syndrome(61.9%)> intestinal dampness and hot muggy syndrome(20.6%)> Qi stagnation and blood stasis syndrome(8.2%),> Spleen deficiency and dampness and hot muggy syndrome(5.2%)> Yang Deficiency syndrome(2.1%)= Spleen Deficiency and Liver Qi stagnation syndrome(2.1%).Syndrome distribution of non-adenomatous polyps from high to low were: spleen insufficiency and wet syndrome(55.4%)> intestinal dampness and hot muggy syndrome(25.9%)> Qi stagnation and blood stasis syndrome(9.8%)> Spleen deficiency and dampness and hot muggy syndrome(3.6%)> Yang Deficiency syndrome(2.7%)= Spleen Deficiency and Liver Qi stagnation syndrome(2.7%).The trend of both syndrome type distribution was completely agreement with the overall syndrome type distribution trend.Men were more likely to get intestinal dampness and hot muggy syndrome than women(P <0.05).Syndromes distribution of all ages was basically the same(P> 0.05).Patients with more than three polyps tended to have a spleen insufficiency and wet syndrome or to have a qi stagnation and blood stasis syndrome comparing with a single polyp(P <0.05).And 2-3 polyps tended to form a liver depression and spleen deficiency syndrome.ConclusionOld age,Hp infection,smoking,peptic ulcer are the risk factors of colorectal polyps.Improving diet cleanliness and exercising are protective factors in the pathogenesis of colorectal polyps,and it is better to maintain a certain exercise intensity.The most common syndrome of colorectal polyps is a mixture of deficiency and excess.The distribution of different pathological types of syndromes are basically the same.The intestinal dampness and hot muggy syndrome is more common in men,and spleen insufficiency and wet syndrome,qi stagnation and blood stasis syndrome and liver depression and spleen deficiency syndrome are more common in patients with multiple polyps.
