The Study Of The Regular Pattern Of Syndrome And Treatment On Treated Idiopathic Membranous Nephropathy By Prof.lv Hongsheng

Objective:Through collecting and sorting out the medical cases of treating idiopathic membranous nephropathy(hereinafter referred to as membranous nephropathy)in Professor Lv Hongsheng’s outpatient department,this thesis aims to combine the rules of syndrome treatment and specialty medicines,to explore the characteristics of syndrome differentiation and treatment,the thinking of diagnosis and treatment,the laws of drug use,to refine Professor Lv Hongsheng’s dialectological diagnosis and treatment thinking,medication laws and specialty drugs for diagnosing and treating membranes nephropathy,and to summarize the experience of Professor Lv Hongsheng to treat membranous nephropathy.Methods:1.Gathering and sorting out the medical cases of patients with idiopathic membranous nephropathy from March 2011 to February 2018 who were treated in the outpatient department of Prof.Lv Hongsheng,including the patient’s basic conditions,diagnosis and treatment process,four consultations,laboratory tests,treatment and outcomes,and then keeping them into files by taking pictures;2.By using the Microsoft Office Excel 2017 software to build a database of patient cases,the original data is standardized and entered into the “Ancient and Modern Medicine Cloud Platform V1.4.1” software(developed by the Institute of Traditional Chinese Medicine Information of China Academy of Chinese Medical Sciences).After the completion of case data entry,through the “Data Mining Analysis” function in the “Ancient and Modern Medicine Cloud Platform V1.4.1” software,descriptive statistical methods(Descriptive Statistic)are used for preliminary statistical analysis like frequency data analysis and frequency analysis.Data Association Rules such as support-confidence association rules and cluster analysis are used to make the association analysis on the data;3.By analyzing the rule of syndrome and treatment of membranous nephropathy,a deep study is conducted on Professor Lv Hongsheng’s unique drug prescription,medication characteristics and regularity in the treatment of membranous nephropathy.Results: 1.In this study,144 cases and 1796 visitors are finally adopted,including 89 males(61.81%)and 55 females(38.19%).The male to female ratio is 1.62:1.The youngest is 19 years old,and the oldest is 78 years old.The average age is 44.65±14.29 years old.2.The membranous nephropathy patients from more to less are membranous nephropathy stageⅡ,membranous nephropathy stageⅠ,membranous nephropathy stage Ⅰ-Ⅱ,membranous nephropathy not clearly staged,membranous nephropathy stage Ⅱ-Ⅲ.3.By using descriptive statistical methods for frequency date analysis and frequency analysis,symptoms of membranous nephropathy are mainly manifested as follows: more urinary bubbles,different levels of edema in lower limbs,face,and scalp,divine fatigue,abnormal bowel movements,nocturia,and back pain,abnormal sleep;Tongue vein mainly manifested as reddish or red tongue,thin white fur,smooth pulse,and dark tongue,greasy or white,slow pulse is also more common.4.By using descriptive statistical methods for frequency date analysis and frequency analysis of TCM syndromes,the following information is obtained: The highest frequency TCM syndromes are spleen and kidney deficiency syndrome,kidney deficiency syndrome,water-sweeping syndrome,kidney deficiency syndrome,damp heat syndrome,blood stasis syndrome,liver-kidney yin deficiency syndrome,exogenous wind evil syndrome,hot and humid injecting syndrome,lung qi non-demonstration,Yin deficiency,Huoxue syndrome,Liver depression syndrome,Restlessness syndrome,Fengshui syndrome,etc.;The most using frequency of TCM are Shanyurou,Fuling,Huangqi,Yumixu,Chaobaizhu,Yinyanghuo,Chuanniuxi,Sangjisheng,Fangji,Dangshenpian,Chuanxiong,Danshen,Qianshi,etc.The most using amount are Fuling,Huangqi,Yumixu,Sangjisheng,Danshen,Qianshi,Dongguapi,Shouwuteng,Yiyiren,etc.;The most frequency of treatments for membranous nephropathy are mainly: invigorating spleen and kidney,kidney controlling actions,removing stasis and alleviating water retention,promoting blood circulation and removing blood stasis,invigorating the kidney and activating blood circulation,clearing heat and transforming dampness and so on.5.By using cluster analysis,Professor Lv Hongsheng uses 10 group medicines,namely: Huangqi + Fuchaobaizhu,Chuanniuxi + Sangjisheng,Danshen + Chuanxiong,Dihaung + Mudanpi,FUpian + Dongguapi,Gancao+ Caodoukou,Banxia + Chenpi,Shouwuteng+ Chaosuanzaoren,Huangbo+ Shiwei,Buguzhi+ Chaoshanyao.6.Yi Quan capsule is used 458 times,accounting for 26% in the 1796 clinical times.7.By using frequency statistics,it is obtained that the drugs with the frequency of four gas from more to less were mild,warm,small warm,tepor,cold,small cold,great hot,hot.Great cold medicines is the least.The five taste frequency from more to less are sweet,bitter,pungent,acid,tasteless,puckery,small bitter,salty,small sweet.Small pungent medicines is the least.The order of the most frequently from more to less are liver meridian,spleen meridian,kidney meridian,lung meridian,and heart meridian,which is the least of the three focal points.8.By using the support-confidence association rule mining method,after conducting the association analysis of Professor Lv Hongsheng’s TCM for the treatment of membranous nephropathy,there is a maximum probability of spleen and kidney deficiency syndrome and Stasis water alternating knot appearing at the same time.After conducting the association analysis of traditional Chinese medicine commonly used,it can be concluded that the probability of occurrence of both Huangqi and Fuchaobaizhu at the same time is the highest.Conclusion: 1.Lv’s treatment of membranous nephropathy is based on syndrome differentiation and treatment.He believes that the pathogenesis includes both virtual and real.It is virtual-based,and is mostly shown as deficiency of the spleen and kidney.Standardization is commonly seen in wetness,dampness,blood stasis,and the probability of the occurrence of two deficiency syndromes of water-sparing knots → spleen and kidneys occurring at the same time is the greatest.The governing method is mostly based on the principle of protecting the spleen,strengthening the spleen and tonifying the kidneys with the assistance of removing stasis and alleviating water retention,dissipating coldness and alleviating fever.2.In the treatment of membranous nephropathy,traditional Chinese medicines used frequently by Prof.Lv are: Shanyurou,Fuling,Huangqi,Yumixu,Chaobaizhu,Yinyanghuo,Chuanniuxi,Sangjisheng,Fangji,Dangshenpian,Chuanxiong,Danshen,Qianshi,etc.There is the maximum probability of Fuchaobaizhu→ Huangqi and spleen occurring at the same time.10 group medicines Professor Lv Hongsheng likes to use are: Huangqi + Fuchaobaizhu,Chuanniuxi + Sangjisheng,Danshen + Chuanxiong,Dihaung + Mudanpi,FUpian + Dongguapi,Gancao+
