Objective:1.Analyze the effect of different factors on the expression and positive rate of serum MG7-Ag;2.To analyze the similarities and differences of serum MG7-Ag expression and positive rate in different gastric mucosal populations;3.Determine the diagnostic value,sensitivity and specificity of serum MG7-Ag in the diagnosis of gastric precancerous lesions and evaluate its diagnostic value.Methods:Randomly selected from June 2016 to March 2017 at the Yan’an University Affiliated Hospital for gastroscopy,pathological examination,Helicobacter pylori detection(14C breath test)and serum MG7-Ag test,As well as further testing for Helicobacter pylori,gastroscopy,and pathology in the screening program for teachers of gastrointestinal cancer in Yan’an,According to histopathological results,217subjects included in the study were divided into five groups:active mucosal inflammation,chronic mucosal inflammation,mucosal atrophy,intestinal metaplasia,and intraepithelial neoplasia.Analysis of its clinical data:1.Statistics of the subject’s gender,age,Helicobacter pylori infection status,whether there is a family history of cancer,whether or not other digestive system diseases and other general information;2.Analysis of different gastric mucosal MG7-Ag expression levels and the similarities and differences of the positive rate;3.Through multiple linear regression analysis of serum MG7-Ag levels related factors;4.Draw serum MG7-Ag diagnosis of gastric precancerous lesions The characteristic curves of the MG7-Ag in the diagnosis of gastric precancerous lesions and the sensitivity and specificity were used to evaluate the diagnostic value.According to the 2012 version of the“Consensus Opinion of Chronic Gastritis in China”diagnostic criteria,gastroscopy and pathological diagnosis were performed.In the present study,SPSS 22.0 was used for data analysis.The serum MG7-Ag expression level was a measurement data.The normality test of each group of data was in accordance with the normal distribution,expressed as the mean±standard deviation.The t test was used to compare the two groups.One-way analysis of variance was used for comparison between groups.Count data is expressed in%,using the X2test.Draw the characterization curve of serum MG7-Ag in the diagnosis of precancerous lesions,and use the critical value when the index is defined as the maximal index=sensitivity+specificity-1.Multiple linear regression analysis was used to assess factors that influence serum MG7-Ag expression levels..The P value of the x-card X2 test was Bonferroni-corrected to<0.05/group with statistical significance,and the remaining statistical methods were P-value<0.05 with statistical significance.Results:1.Of the 217 subjects included in this study,111 were males(51.15%),14 were MG7-Ag positive(12.61%),106 were females(48.85%),and serum MG7-Ag positive were 10 Example(9.43%).There were 96 cases(44.24%)in the youth group(16-44years old),2 cases(1.04%)with serum MG7-Ag positive,121 cases(55.76%)in the middle-aged group(≧45 years old),and serum MG7-Ag positive persons 22 Example(18.18%).Hp positive was detected in 132 cases(60.83%),among which serum MG7-Ag positive was 21 cases(15.91%),Hp negative was detected in 85 cases(39.17%),serum MG7-Ag positive was 3 cases(3.53%).Fifty-two subjects(23.96%)had first-degree relatives with a history of cancer,and four cases(76.92%)had positive serum MG7-Ag.89 cases(41.01%)were complicated with other digestive diseases,and 7cases(78.65%)were positive for serum MG7-Ag.2.The effects of different factors on the expression and positive rate of serum MG7-Ag were compared.There was a significant difference in the expression and positive rate of serum MG7-Ag between subjects with different ages and H.pylori infection status(P<0.05).The serum levels of MG7-Ag in the subjects with family history of tumor and no family history of tumor were significantly different(P<0.05).The positive rate of serum MG7-Ag between the two groups was not significantly different(P>0.05).3.The expression and positive rate of serum MG7-Ag in different gastric mucosa were statistically significant(P<0.05).Further comparisons between the two groups showed that there was no significant difference in serum MG7-Ag expression between active inflammation group and chronic inflammation group,atrophy and intestinal metaplasia group(P>0.05).The difference among the other groups was statistically significant.(P<0.01).The positive rates of serum MG7-Ag in patients with intraepithelial neoplasia were significantly different from those in active inflammation group and chronic inflammation group(P<0.01).There was no significant difference in the positive rate of MG7-Ag between different gastric mucosal lesions(P>0.01).The serum MG7-Ag expression level and positive rate in patients with precancerous and non-precancerous lesions in the stomach were significantly different(P<0.05).4.Multiple linear regression analysis of factors affecting serum MG7-Ag levels resulted in age,Helicobacter pylori infection,and gastric precancerous lesions as factors that influence serum MG7-Ag levels.5.Plot the characteristics of serum MG7-Ag in the diagnosis of gastric precancerous lesions.The area under the curve and the 95%confidence interval for serum MG7-Ag are:0.735(0.619-0.851).The Yodner index was max=0.446.The cutoff value of serum MG7-Ag 4.5ng/ml was taken as the critical value.The sensitivity was 59.3%and the specificity was 85.3%.Conclusion:1.Age,Helicobacter pylori infection,and gastric precancerous lesions are related factors that affect the expression of serum MG7-Ag.2.The level of serum MG7-Ag is closely related to the degree and nature of gastric mucosal lesions.With the increase in the severity of gastric mucosal pathological diagnosis,serum MG7-Ag basically showed a rising trend.Detecting changes in serum MG7-Ag expression can predict the progression of gastric mucosal lesions to some extent.3.Preliminarily determine the critical value of serum MG7-Ag in the diagnosis of gastric precancerous lesions,but when it is used as a screening test for gastric precancerous lesions,the rate of missed diagnosis may be higher,resulting in the accuracy of the screening test and the diagnostic efficiency.influences.
