Objective: To understand the operation status and performance management of township health centers in Changting County,Longyan City,Fujian Province;to discuss the reasons for the success of the primary health care reform in Changting County and the related factors affecting the performance management;and to propose the promotion of township health The policy recommendations for the sustainable development of the hospital provide reference for the government to further promote the development of rural hospitals in remote mountainous areas.Methods: In this study,qualitative and quantitative research methods were used to collect survey data,routine registration report data,and related literature data.Descriptive statistical methods and multiple linear regression methods were used to analyze the operation status,human resources status,and performance management factors of township health centers;the analysis of the distribution of human resources in township health centers was based on the distribution of the population using the Lorenz curve and the Gini coefficient.Sex;using health service demand method to measure the demand of medical staff in township hospitals;using DEA method to analyze the operating efficiency of township health centers;and using multiple linear regression model to evaluate the impact factors of performance management plans for township hospital staff Conduct the analysis.Results:In terms of infrastructure construction,the infrastructure construction of township health centers in Changting County has developed to a certain degree.The average building area per courtyard is much higher than the average level in China.There is actually an increase of 28.70% in the number of beds compared to 2012,and 18 hospitals have ten thousand In terms of basic medical service capabilities,in 2016,thenumber of outpatient and hospital visits in township and township hospitals increased by 71.62% and 91.87%,respectively,compared with 2012.The average number of outpatient and emergency visits,hospitalizations,and hospital beds were all far higher than those in China.Average level;In the allocation of human resources,the total human resources in 2012-2016 showed a trend of increase,but the health of technicians and practicing(assistant)physicians was poor according to the population distribution.In 2016,technicians and practitioners(The Gini coefficient of the distribution of physicians and registered nurses according to population was 0.444,0.455,and 0.390 respectively.In the evaluation of supply and demand sides,the total number of workers satisfied with the work accounted for 73.96% of the surveyed population,and the outpatients’ various items of medical services Satisfaction appraisal Satisfaction rate is about 84%;in the aspect of economic operation,the income of township hospitals in2012-2016 With the increase,health centers are operating efficiently.In 2016,the overall efficiency and technical efficiency of most township and township hospitals are effective.In the performance management system,the staff of health centers highly evaluates the performance management system,and multiple linear regression analysis results show that performance management The score of the program evaluation is related to the job category,working years,the ratio of performance-related wages to monthly wages,performance-based pay,whether to participate in the formulation of performance plans,and whether the organization conducts training and publicity of performance plans.Conclusion: In recent years,the basic medical reform in Changting County has achieved remarkable results: First,the scale of township and township hospitals has achieved certain development;Second,the ability of basic medical services has been continuously improved;Third,the total amount of human resources for health has increased,but the fairness of the allocation remains to be further improved.Fourth,the hospitals are operating efficiently;Fifth,the supply and demand sides are highly satisfied.In addition,through the research on performance management,it is found that staff members are highly satisfied with performance management program evaluation;staff job categories,working years,proportion of performance-based wages,participation performance plan development,and performance management program training and advocacy meetings within the unit.Affects the performance management program evaluation results.Based on the study of basic health care reform in Changting County,we propose several suggestions for sustainable development of health centers in remote mountainous areas: First,we must give play to the role of the government;Second,we must activate the institutional mechanism for the management and operation of health centers;The fourth is to pay attention to the organic combination of medical and health protection;the fifth is to establish a scientific performance appraisal system;and the sixth is to strengthen the contingent of qualified personnel.
