Clinical Significance Of Joint Detection Of Serum CEA And CA242 Tumor Markers In Patients With Lung Adenocarcinoma

Objective: The purpose of this study was to discuss the application value of single detection and joint detection of tumor markers about carcinoembryonic antigen(CEA)and 242 Carbohydrate antigen(CA242)in the diagnosis of lung adenocarcinoma.Methods: 1 Objective: to study the medical records of patients with lung adenocarcinoma in the second hospital of shanxi medical university from December 2015 to December 2017.2 Index analysis: record and statistics baseline indicators such as gender,age,height,weight,smoking history,pathological results(immunohistochemical,degree of differentiation,pathological staging),clinical stage,pleural effusion(chest colour doppler ultrasound clear that pleural effusion > 100 ml)and tumor associated antigen(CEA,CA242,CA199,CA125,CA153),analysis the correlation of lung adenocarcinoma and gender,smoking history,pleural effusion,tumor related antigen.To determine the specificity and sensitivity of the joint detection of tumor markers,carcinoembryonic antigen(CEA)and glycoprotein antigen 242(CA242)in the diagnosis of lung adenocarcinoma.3 Statistical analysis: Analysis by SPSS 24.0 statistical software,measuring information by the mean + /-standard deviation,said between groups to compare with the comparison to the two sample mean t test or analysis of variance,categorical data analysis using the chi-square test or Fisher’s exact probability method,The risk factors of lung adenocarcinoma were analyzed by Logistic regression,and the working curve of the subjects was drawn to determine the efficacy of CEA and CA242 in the diagnosis of lung adenocarcinoma.P < 0.05 had statistical significance.Results: 1 Among the cases,there were 135 females and 96 males,Women had 39 more patients than men,accounting for 16.9 percent of the total,and women had higher rates than men.2 Smokers with more than one year was accounted for 84.4% of all male patients,significantly higher than the 31.1% of female patients,found that male patients with lung adenocarcinoma with disease patients is related to smoking(P < 0.001).3 The incidence of malignant pleural effussion in advanced lung cancer is as high as 60% and often progressive.The pleural effussion was positively correlated with pathological staging(P<0.001).It can be found from ROC curve that AUC value is 0.86,P<0.001,and pleural effussion can indicate the further development of disease,and it has a clear indication of the value of advanced lung adenocarcinoma.4 The positive rate of CEA in patients with lung adenocarcinoma accounted for about 81.0%,the positive rate in patients with non lung adenocarcinoma accounted for about 73.0%;the positive rate of CA242 in patients with lung adenocarcinoma accounted for about 74.9%,the positive rate of CA242 in patients with non lung adenocarcinoma accounted for about 44.2%,the specificity is stronger than CEA,but the sensitivity is poorer.Through the ROC test,the AUC value of CEA was 0.54,the AUC value of CA242 was found to be 0.66,the AUC value of CA199 was 0.57,the CA125 value was 0.56,and the CA153 value was 0.56,all of them was statistically significant(P<0.05).The sensitivity of CA242 was highest but the specificity was poor,and it was not significant to identify the types of lung cancer alone.5 When CEA and CA242 were positive,AUC=0.75(P<0.001);When CEA and CA199 were positive,AUC=0.58(P=0.005);When CEA,CA242 and CA199 were positive,AUC=0.59,CEA,CA242,CA199 and CA125 were positive,AUC=0.56(P<0.05).When CEA,CA242,CA199,CA125 and CA153 were positive,AUC=0.56(P<0.05).The joint detection of CEA and CA242 has certain clinical value for the identification of lung cancer types.Conclusion: 1 We found that CEA was highly sensitive and had low specificity;CA242 has obvious advantages in the identification of lung adenocarcinoma and other lung cancer,but it cannot be used as a screening index.The expression of CA199,CA125 and CA153 in lung adenocarcinoma is significantly concentrated on advanced tumor,and the significance of the identification of lung adenocarcinoma is small.2 In multiple combinations,the combination of CEA and CA242 has obvious advantages,although its sensitivity is not very high,but still can help us to identify the pathological type of lung cancer in the clinical diagnosis and treatment.
