Objective:Compare the clinical effects by the trans-areolar incision and conventional trans-subaxillary incision of thoracoscopic sympathetic chain transection in the treatment of primary palmar hyperhidrosisMethods:The clinical data of 80 patients of primary palmar hyperhidrosis who received the treatment of thoracoscopic sympathetic chain transection by the trans-areoar incision(The trans-areoar incision group)and the clinical data of 95 patients of primary palmar hyperhidrosis who received the treatment of thoracoscopic sympathetic chain transection by the conventional trans-subaxillary incision(The trans-subaxillary incision group)between January 2015-January 2016 were retrospectively collected and compared.The differences in complication rate,recurrence rate,hand statue,hospitalization day,hospital cost,operative time,patient satisfaction and VAS between the two groups were also compared.Results:The average operation time of the trans-areoar incision group was(13.28±4.47)min,which was significantly shorter than that of the trans-subaxillary incision group(18.67±8.80)min.The difference between the two groups was statistically significant(P<0.05).There was no statistical difference in complication rate,recurrence rate,hand statue,hospitalization day,hospital cost,patient satisfaction and VAS between the two groups.Conclusion:Thoracoscopic sympathetic chain transection through the trans-areoar incision and conventional trans-subaxillary incision is both safe and effective for the treatment of primary palmar hyperhidrosis.Compared with the trans-subaxillary incision group,the trans-areoar incision group had a shorter operative time and better postoperative wound cosmetic results.However,the trans-areoar incision is more suitable for the thin chest and body wasting male patients,not suitable for women(because of the developed female breasts,abundant blood supply and the chest wall tunnel that easily bleeds),and the trans-subaxillary incision is widely applicable to all patients.
