ObjectivesTo understand the current status of chronic diseases in middle-aged and elderly people in China, and to empirically analyze whether childhood socioeconomic status(SES)can make some influence to the prevalence of chronic diseases in middle-aged and elderly people.Combining relevant research results at home and abroad, provide effective prevention for chronic diseases in China,Put forward policy recommendations to prevent the risk of chronic diseases based on empirical evidence.MethodsThis study mainly uses quantitative research method.First of all, through literature review and theoretical research, constructs the theoretical framework of research. After that, descriptive statistical analysis was used to make a description of the basic situation of the related data of the CHARLS project, and then through the single factor test and Logistic regression analysis on the relationship between the SES index of childhood and the three chronic diseases:cardiopathy, diabetes or blood glucose increase and hypertension.Results1.The prevalence rates of cardiopathy, diabetes, and hypertension in the interviewed population were 11.57%,5.77%,and 42.64%respectively.Univariate analysis showed that the prevalence of cardiopathy, diabetes and hypertension was correlated with average age, gender, regional distribution, education level, per capita income, and type of place of residence(P<0.05).The prevalence of cardiopathy and hypertension is also related to marital status respectively.2.The results of Logistic regression showed that the SES in childhood had a direct influence on the risk of cardiopathy in the middle-aged and elderly people.Compared with the children's father did not receive regular education, mother farming, family self-assessment was the same as others and the main residence was in the rural in childhood, the children's father received regular education in childhood(OR=1.286),mothers engaged in non farm work(OR=1.563),poor family economic status in childhood(OR=1.179),and lived in urban(OR=1.651)had a higher prevalence rate of cardiopathy.Childhood SES had a direct influence on the risk of diabetes in middle-aged and elderly people, compared with the children with poor economic status in the family, the child lived in good family economic condition in childhood had higher risk of diabetes(OR=1.433).There was no direct relationship between the childhood SES and the risk of hypertension(P>0.05).ConclusionsThe prevalence rate of chronic diseases is high in Chinese 45-year-old and older people. There is also a large proportion of people with bad behavioral lifestyles. We can find a correlation between current SES and chronic diseases. In the analysis of the influence by childhood SES to the chronic disease risk,we found that middle-aged and elderly people with mother's occupation is non-agricultural,father's education level is high, living in the city, and the family's economic status is poor during childhood are more likely to suffer from cardiopathy;middle-aged and elderly people with good family economics during childhood are more likely to suffer from diabetes;childhood SES has no direct influence on the risk of hypertension in middle-aged and elderly people. The effects of childhood SES on the three chronic diseases are different. It may be the result of the interaction of the social environment, biological genes, and the mechanism of the disease itself.Suggestions1.Popularize healthcare education and promote healthy behavioral lifestyle.2.Reduce the consumption of tobacco and alcohol, establish a series of strict tobacco and alcohol control regulations.3.Ensure the healthy development and social welfare priority of children.4.Strengthen primary health infrastructure and improve the equity of healthcare.5.Promote the health of all citizens, establish a chronic disease prevention and control system for the entire life cycle.
