Objective: Endoscopic retrograde cholangiopancreatography(ERCP)and related techniques play an important role in the diagnosis and treatment of biliary and pancreatic diseases.As China's aging society continues to deepen,elderly patients as a common cause of common bile duct stones,biliary and pancreatic tumors,ERCP for the diagnosis and treatment of biliary diseases in elderly patients have been widely used by clinicians and patients.Attention and recognition.However,there are more and more concerns about the effectiveness and safety of related technologies such as ERCP for elderly patients over 80 years old.The aim of this study was to evaluate the efficacy and safety of ERCP in elderly patients over 80 years of age by comparing the use of ERCP in elderly patients of different ages.Methods: Data from 383 elderly patients who underwent ERCP in our hospital from January 2018 to December 2018 were collected for analysis.Patients over the age of 80 were enrolled in the advanced age group(n=86);patients aged 60-79 years were established as the control group(n=297).Patient comorbidities,case data,clinical diagnosis(eg,presence of biliary calculi,biliary stenosis and malignancy),ERCP surgical details(treatment,technical success rate,operative time),retrospective analysis of ERCP-related complications and mortality.Results: A total of 383 patients were enrolled,the patients were associated with underlying disease,and the difference was statistically significant(P < 0.05).There were statistically significant differences between the two groups in the previous cholecystectomy and gallstones.Thesuccess rate of the two groups compared with the intraoperative related conditions such as the length of surgery,the success rate of intubation,etc.,the results showed no significant difference.There was no significant difference in the incidence of postoperative complications related to ERCP between the two groups.In the elderly group,one patient had a sudden unconsciousness,decreased blood pressure,and decreased heart rate during the surgery,and died after being rescued.One case of postoperative infection increased the severity of infection and respiratory failure.In the control group,two patients were caused by poor biliary drainage,increased bile duct inflammation,and liver failure.No patients with gastrointestinal perforation were seen in either group.Conclusion: The application of ERCP in elderly patients aged 80 years and older is safe and effective.The incidence of postoperative complications without ERCP is not significantly increased.Old age is not a contraindication to surgery for ERCP,but it is necessary to comprehensively evaluate the general condition of the patient before surgery,and improve related examinations,and then perform surgery after the original underlying disease is stable.
