Clinical Study On The Treatment Of Late Knee Osteoarthritis With Dense Silver Needle Heat Conduction

Objective:At present,there are many conservative treatment methods for advanced knee osteoarthritis,but the curative effect is not ideal,although oral drugs and intracavitary injection of sodium hyaluronate can alleviate knee osteoarthritis to a certain extent.But most patients suffer from knee joint pain and dyskinesia resulting in a low quality of life.Therefore,it is necessary to find an economical and effective way to alleviate the pain of patients with advanced knee osteoarthritis.Clinically,we observed that silver needle heat conduction is effective in the treatment of advanced knee osteoarthritis,so in this study,intensive silver needle heat conduction therapy and oral celecoxib therapy were used to observe the clinical efficacy of the two groups.Through the comparison between silver needle enhanced thermal conduction therapy and oral celecoxib therapy,the efficacy and safety of silver needle enhanced thermal conduction therapy were evaluated while analyzing the mechanism of action.It provides a new and effective non-operative treatment for the treatment of advanced knee osteoarthritis.Metholds : In this study,60 patients with advanced knee osteoarthritis were selected from Ruikang Hospital affiliated to Guangxi University of traditional Chinese Medicine and Airport Hospital from February 2018 to February 2019.a prospective randomized controlled study was conducted.The patients were equally divided into two groups.In the treatment group,the knee joint was punctured with silver needle,the first group was located at the suprapatellar bursa and subpatellar fat pad,and the second group was near the soft tissue around the knee joint,according to the condition of the disease.Choose in tibia,fibula collateral ligament,adductor tube,lateral femoral muscle and so on,from proximal to distal oblique prick to periosteum attachment,according to the patient’s tolerance,according to the location.Usually 3 times of treatment,each treatment interval of one week.The control group was treated with oral celecoxib capsule 0.2g,once a day,21 days as a course of treatment.After 1 month and 3 months of treatment,VAS pain score,Lysholm function score of knee joint,therapeutic effect and laboratory examination of interleukin(IL-1)and tumor necrosis factor α(INF-α)were compared and evaluated between the two groups.Results:1.According to the results of the comparison and analysis of VAS pain scores before and after treatment between the two groups,both the intensive silver-needle heat conduction group(treatment group)and the oral celecoxib capsule group(control group)had the effect of alleviating pain,but the degree of improvement of the former was significantly better than the latter(P<0.05),and the analgesic effect of long-term relief was particularly significant.2.According to the results of Lysholm knee joint score before and after treatment of the two groups of patients,the intensive silver-needle heat conduction group(treatment group)and the oral celecoxib capsule group(control group)improved the pain and flexion and extension of the knee joint of the patients,but the former was better than the latter(P<0.05),and the long-term effect was particularly significant.3.Compared with oral celecoxib,the total effective rate of silver-needle heat conduction therapy between the two groups after 1 month and 3 months of treatment was higher than that of oral celecoxib,both of which showed a significant difference(P<0.05),suggesting that the efficacy of silver-needle heat conduction was better.4.Two groups of patients before and after treatment in joint fluid content of TNF and IL-1-alpha comparative analysis shows that compared with group before treatment,whether it is or after treatment between the two way more,all P < 0.05,showed that the difference was statistically significant,patients with two groups of treatment can reduce the KOA,IL-1 content in knee joint fluid,but the curative effect of treatment group is better than the control group.Conclusion:Intensive heat conduction silver needle therapy and oral celecoxib capsules treatment knee osteoarthritis were late curative effect,but more precise intensive heat conduction effect of silver needle,the long-term curative effect is more better than that of oral drug,can significantly reduce joint fluid IL-1 beta and TNF alpha content,reduce joint inflammation around the KOA patients,to ease the pain of patients,improve patient quality of life,is worth popularization and application in clinic.
