Objective: To detect intracranial arterial hemodynamic parameters in patients w ith acute ischemic cerebral infarction in different clinical subtypes by transcranial D oppler(TCD),and to explore intracranial arteries of acute ischemic stroke in differ ent OCSP subtypes.Blood flow changes and their characteristics.Methods: A total of 318 hospitalized patients with acute ischemic cerebral infarction were enrolled i n Xinjiang.According to clinical manifestations,patients were divided into four gro ups according to the Oxfordshire Community Stroke Project(OCSP): complete ante rior circulation infarction(TACI)group,some before Circulatory infarction(PACI)group,posterior circulation infarction(POCI)group and lacunar infarction(LACI)g roup.All patients underwent transcranial Doppler(TCD)to assess intracranial arteri al blood flow within 24 hours of onset.The abnormal rate of blood flow velocity in each blood vessel was calculated,and the hemodynamic results of patients with different OSCP subtypes were observed.Results: A total of 318 patients were enro lled,including 145 males and 173 females,with an average age of(58±5.6)years,grouped into 57 patients(18%)in the TACI group,63 patients(20%)in the PAC I group,and 104 patients in the POCI group(33%),94 cases(29%)in the LACI group.There was no significant difference in the proportion of patients with hype rtension,coronary heart disease history,smoking history,and drinking history in ea ch group(P>0.05).The abnormal rate of arterial blood flow in each group was 68%,75%,62%,and 29%,respectively,and the difference was significant(P<0.05).In the group,49%,47%,27%,and 15% of patients had abnormal blood flow in the middle cerebral artery(MCA),and 78% of them had a slower flow rate(P<0.05).There was no significant difference in the proportion of patients with abnormal blood flow in the anterior cerebral artery(ACA),posterior cerebral artery(PCA),and internal carotid artery end stage(TICA)(P>0.05).The abnormal rates of basil ar artery(BA)and vertebral artery intracranial segment(I-VA)in various groups w ere 13%,6%,34%,and 26%,respectively(P<0.05).Conclusion: There are signific ant differences in intracranial arterial hemodynamics between the four subtypes of a cute ischemic cerebral infarction.It is proved that the abnormality of intracranial ar terial blood flow is partially related to the clinical subtype of OCSP.TCD examina tion has a positive guiding role in the early assessment of acute cerebral infarction,diagnosis and prognosis.
