Objective: To observe the clinical effect of acupuncture on primary open-angle glaucoma with optic nerve damage,and to explore the efficacy and clinical advantages of acupuncture on primary open-angle glaucoma with optic nerve damage,so as to provide new ideas and methods for the clinical treatment of primary open-angle glaucoma with optic nerve damage.Methods:From November 2017 to December 2018,we visited the First Affiliated Hospital of Yunnan University of Traditional Chinese Medicine.Western medicine diagnosed primary open angle glaucoma optic nerve damage,Chinese medicine diagnosed glaucoma as liver stagnation and qi stagnation syndrome.Sixty patients(120 eyes)who met the criteria for inclusion of the project were randomly divided into control group and experimental group,30 cases(60 eyes)in each group.The control group was treated with routine intraocular pressure lowering drugs and Mecobalamin dispersible tablets orally,including early intraocular pressure control(< 18 mm Hg),advanced stage(<15mm Hg)and late stage(<12mm Hg).The experimental group was treated with acupuncture on the basis of the control group,and the acupoints were selected as follows:bilateral upper eyeball bright,accumulating bamboo,pupils,silk bamboo hollow,and scalp needle:sun,bilateral wind pool,Baihui,Sishensheng;Body acupuncture is:bilateral Sanyinjiao,Zusanli,Taichong,Hegu,Guangming and Lieqian.Acupuncture is a course of treatment with 14 days interval of 6 days.Continue the next course of treatment,a total of 3 courses of treatment.The changes of visual acuity,visual field,P-VEP,OCT and TCM symptom scores were observed at the time of admission and after the course of treatment.Result: After three courses of clinical observation experiment,the indexes were analyzed under SPSS22.0.The results showed that:(1)There was no significant difference in age,sex,course of disease,visual acuity,visual field,P-VEP,OCT and TCM symptom scores between the control group and the experimental group before treatment(P >0.05).(2)Visual acuity comparison: control group: there was no significant difference in visual acuity before and after treatment(P>0.05);experimental group:after treatment,visual acuity improved compared with before treatment,the difference was statistically significant(P<0.05).There was significant difference in visual acuity between the two groups after treatment(P < 0.05),and there was significant difference in effective visual acuity between the two groups after treatment(P < 0.05).(3)Visual field comparison: control group: there was no significant difference in visual field score and average visual field defect before and after treatment(P>0.05);experimental group: after treatment,visual field score and average visual field defect decreased compared with before treatment,the difference was statistically significant(P<0.05);two groups of patients after treatment visual field score,average visual field defect comparison,the difference was statistically significant(P<0.05);After treatment,the visual field effect analysis of the experimental group was better than that of the control group,and the difference was statistically significant(P< 0.05).(4)P-VEP comparison:The P100 latency and amplitude of P-VEP in the two groups after treatment were changed compared with those before treatment,and the difference was statistically significant(P<0.05).After treatment,the latency and amplitude of P100 in the experimental group were shorter and higher than those in the control group(P<0.05).(5)Comparisons of TCM symptoms:The total score of TCM symptoms in the two groups decreased after treatment and before treatment,with statistical significance(P<0.05);after treatment,the total score of TCM symptoms in the experimental group was significantly lower than that in the control group,with statistical significance(P<0.05).(6)Comparison of TCM symptoms:After treatment,the total score of TCM symptoms in the two groups was lower than that before treatment,with statistical significance(P<0.05);after treatment,the total score of TCM symptoms in the experimental group was significantly better than that in the control group,with statistical significance(P<0.05).Conclusion:Acupuncture has a good clinical effect on primary open-angle glaucoma with optic nerve damage.It can effectively improve the visual function of patients,improve their visual acuity,decrease the average visual field defect,shorten the latency of P100 wave,increase the amplitude,delay the decrease of RNFL average thickness,and play a protective role in optic nerve.It can also improve the symptoms of headache,eye pain,depression,dry mouth and other symptoms,alleviate liver depression and Qi stagnation.
