Analysis Of Risk Factors For Ischemic Stroke In Patients With Nonvalvular Atrial Fibrillation

Objective:To nvestigate the clinical data of atrial fibrillation(AF)and to collect the related laboratory data as the main content,based on the nonvalvular atrial fibrillation(NVAF)patients in the First Affiliated Hospital of Xinjiang Medical University.Based on the common risk factors of NVAF patients,the prevalence of ischemic stroke(IS)in NVAF was further clarified,and the independent risk factors of NVAF complicated with IS were explored.To analyze the independent risk factors for ischemic stroke with NVAF and to provide the clinical evidence for the prevention of ischemic stroke with NVAF.Methods : Adopting the method of retrospective case-control study,we collected the clinical data and related auxiliary examination index of 743 patients with AF who were at hospital from January 2012 to December 2017 were enrolled.According to the inclusion and exclusion criteria,289 cases were excluded.454 cases were included in the test.According to the presence of ischemic stroke,the group was divided into NVAF group(n=386)and NVAFIS group(n=68).The general clinical data and related auxiliary examination results of patients with non-valvular atrial fibrillation and non-valvular atrial fibrillation complicated with ischemic stroke were analyzed and the clinical features were compared.Univariate analysis was performed using SPSS22.0 software.The results were statistically significant and multivariate two-category unconditional logistic regression analysis was used to determine the significant influencing factors in patients with non-valvular atrial fibrillation and ischemic stroke.The difference was statistically significant at P﹤0.05.Results:The NVAFIS cases were 68.The morbidity rate of the NVAFIS patients was 14.98%.The results of one way analysis showed that: old age(P﹤0.001),LAD(P﹤0.001),D-dimer(P = 0.027),fibrinogen(P = 0.035),hypertension(P =0.005),diabetes(P﹤0.001)were the risk factors in NVAF with ischemic stroke.but the Gender,coronary heart disease,smoking,alcohol consumption,duration of atrial fibrillation,heart rate,SBP,DBP,blood glucose,glycosylated hemoglobin,carotid plaque,TC,TG,LDL,LVEF,hs-CRP,BNP,PLT and NVAFIS No significant correlation.The results of multivarivate logistic analysis showed that: old age(OR=1.043,P=0.019),hypertension(OR=2.319,P=0.032),diabetes(OR=2.148,P=0.008),left atrial enlargement(OR=1.137,P=0.000)were the independent risk factors in NVAF with ischemic stroke.Conclusion:The old age,hypertension,left atrium enlargement and diabetes mellitus were the independent risk factors of NVAFIS.
