Objective:To evaluate the effect of anesthesia index(Ai)in total intravenous anesthesia(TIA)with propofol in the monitor of the depth of anesthesia by comparing with bispectral index(BIS).Methods:Patients underwent elective laparoscopic surgery were monitoring with Ai index and BIS index simultaneously(n=60).Tracheal tube was intubated after general intravenous anesthesia induction,and propofol was continuously transfused to maintain anesthesia.The Ai and BIS values before immediate induction(TO),after immediate intubation(T1),5 minutes after intubation(T2),immediately after pneumoperitoneum(T3),before immediate extubation(T4),5 minutes after extubation(T5),together with the blood pressure(BP),heart rate(HR)of the patients were recorded.Spearman grade correlation analysis was performed between Ai and BIS values and systolic blood pressure(SBP),diastolic blood pressure(DBP)and HR respectively.The significance test of correlation coefficient difference was with u test,and the consistency between Ai index and BIS index was evaluated by Bland-Altman analysis.A P value<0.05 was considered to be statistically significant.Results:1.The values of Ai and BIS at each time point of T0-T5 were basically in the same depth of anesthesia,and there was no significant difference between the values of Ai and BIS(P>0.05).2.Spearman grade correlation analysis showed that the concomitant probability P value was less than 0.01,indicating that Ai value,BIS value and SBP,DBP and HR were significantly correlated;the correlation coefficient r value was basically between 0.3 and 0.5,indicating that Ai value,BIS value and SBP,DBP and HR were low positive correlation.3.The maximum of 6.7%(4/60)of the T0-T5 points is beyond the 95%consistency limit.This difference is clinically acceptable,indicating that the average difference between Ai and BIS values is small,and the systematic errors between the two methods are small.The two methods have good consistency.Conclusions:There is no significant difference between Ai and BIS values when patients are at the same depth of anesthesia under propofol TIA;and the two indices have significant correlation with BP and HR,low positive correlation;the consistency of the two indices is high and both of them can reflect the real-time anesthesia depth of patients objectively.
