| At present,our medical system reform is constantly deepening.The guidance issued by the state has clearly pointed out that public hospitals must follow the principle of public welfare and social benefits.It is necessary to pay attention to both financial indicators and non-financial indicators.Public hospitals need to guarantee medical quality and efficiency,and control the cost of hospitals.Therefore,establishing a performance evaluation system that is consistent with the characteristics of the medical industry is an important part of deepening the reform of the medical and health system,and is of great significance for promoting the development of the public hospital industry.The traditional performance allocation methods used by most departments throughout the year do not reflect the differences in work and labor intensity in different positions of hospitals,which seriously affects the enthusiasm of medical staff.The performance evaluation method based on RBRVS and DRGs is in line with the advanced concept of modern performance management,taking into account the different complexity of the project,and the complexity and seriousness of the disease,thereby giving different points,comprehensive evaluation of the performance of hospital staff.The combination of the two tools can make up for their respective shortcomings and can play a good role in clinical diagnosis and financial management.This paper reviews the related theories of performance evaluation,RBRVS and DRGs,and constructs a theoretical framework based on the integration of RBRVS and DRGs,and completed the system design.Firstly,it expounds the theoretical basis of the research,analyzes the operation of the case hospital,and explains the necessity of introducing RBRVS and DRGs methods.Furthermore,I try to construct a performance evaluation system based on integrated RBRVS and DRGs for RA hospitals.The hospital is divided into clinical departments,medical technology departments,functional departments,and logistics departments to explain the corresponding performance management plans.Among them,the performance calculation method of the clinical department is the most complicated.I further divides it into three groups,then sets the post allocation coefficient,collects the original data,issues the project research table,and determines the RBRVS point value.Finally,the weights and CMI values of the DRGs are obtained,and the corresponding assessment indicators are calculated for different assessment units to allocate performance.In addition,the performance of the medical technology department mainly includes work performance and cost control performance.The assessment methods of the functional department and the logistics department are slightly different,but the basic performance values are set according to a certain proportion of the monthly average performance of the clinical department.This paper uses a combination of theoretical analysis,comparative analysis and case study,based on the research of relevant literature at home and abroad and the in-depth investigation of RA hospitals,to illustrate the combination of RBRVS and DRGs.Analyze the status of performance management in RA hospital and point out the necessity of implementing a new performance evaluation system.Determine the performance evaluation methods and calculation steps of different types of departments,and illustrate the specific acquisition process of RBRVS point values and DRGs weights.Finally,compare the operation of the hospital and the previous situation after the implementation of the new performance evaluation method.The superiority of the performance evaluation system based on the integrated RBRVS and DRGs is illustrated graphically,and proves that this is an effective management solution. |
