The Diagnostic Value Of CA125,HE4,CEA And PLR For Ovarian Tumors

To compare the serum levels of CA125,HE4,CEA and PLR in epithelial ovarian cancer and ovarian benign and borderline tumors,and to assist the differential diagnosis of preoperative diseases.METHODS: 100 cases of epithelial ovarian cancer confirmed by pathology from the First Affiliated Hospital of Xinjiang Medical University from June 2013 to October 2017,100 patients with benign ovarian tumors and 82 patients with ovarian borderline tumors were retrospectively analyzed.The clinical data of the two groups were analyzed,and the serum levels of CA125,CEA,HE4 and PLR were compared between the two groups.RESULTS:(1)The serum CA125,HE4,platelet count and PLR level in the epithelial ovarian cancer group were higher than those in the ovarian benign tumor group,and the lymphocyte count was lower than that in the ovarian benign tumor group.(2)In the logistic regression model,the results showed that CA125,HE4,and PLR contribute to the differentiation of ovarian cancer and benign ovarian tumors.(3)Using ROC curve to compare CA125,HE4 and PLR,the best critical values were 82.1u/ml,69.7pmol/L and 190.24×10 9 /L,respectively.The sensitivity of ovarian cancer diagnosis was 69.0%.82.0%,54.0%,specificity were 89.0%,95.0%,94.0%,and AUC were 0.853,0.872,and 0.798,respectively.The sensitivity of the three combined diagnosis of ovarian cancer was 89.0%,and the specificity was 85.0%.(4)The results of ROC curve analysis showed that HE4 52.2pmol/L was the best cut-off value.When HE4 was greater than 52.2pmol/L,it was epithelial ovarian cancer with AUC of 0.844,sensitivity of 85.0% and specificity of 73.12%.CONCLUSION:(1)Preoperative serum CA125,HE4,and PLR levels in patients with epithelial ovarian cancer are higher than those in ovarian benign tumors and ovarian borderline tumors.(2)CA125,HE4,and PLR are helpful for epithelial ovarian cancer and ovary Identification of benign tumors.(3)The sensitivity and specificity of HE4 in the diagnosis of epithelial ovarian cancer were higher than those of CA125 and PLR.(4)The sensitivity of PLR combined with CA125 and HE4 in diagnosing ovarian cancer was significantly higher than that of a single tumor marker,and the specificity was not significantly reduced.(5)HE4 contributes to the differentiation of epithelial ovarian cancer and ovarian borderline tumors.
