Study On The Distribution Of TCM Syndromes In Phase Ⅲ Of Diabetic Nephropathy And The Relationship With Related Indexes

Objective:To explore the distribution of Traditional Chinese Medicine(TCM)syndromes,and the relation with correlative indicators,through analyzing the clinical symptoms of 80 cases of type 2 diabetic patients with the phase Ⅲ diabetic nephropathy,in order to provide the valuable reference for canonically TCM syndrome differentiation typing of the phaseⅢ diabetic nephropathy.Then it can filtrate the reasonable objective indicators used on clinically estimating the effect of TCM.Methods:This research we collected 80 cases of type 2 diabetic patients with the phase Ⅲ diabetic nephropathy with the method of clinical epidemiology investigation from the endocrine ward of Dalian hospital of Traditional Chinese Medicine between January 2016 and April 2018,then concluded its syndromes stritly and recorded the general condition,body mass index(BMI),hemoglobin A1c(HbA1c),total cholesterol(TC),triglycreide(TG),high density lipoprotein(HDL-C),low density lipoprotein cholesterol(LDL-C),serum cystatin C(CysC),24h urinary protein,serum creatinine(Scr),uric acid(UA)and other objective indicators.All data analyses were processed by SPSS23.0.Results:1.Among the 80 subjects,42 male(52.5%),38 female(47.5),and the ratio of male to famale is about 1.11:1.2.Most of 80 cases of patients are elderly people,accounting for 75%.3.Most of 80 cases of patients’ diabetic courses are less than 20 years,accounting for 63.75%.4.Of the 80 patients in phase Ⅲ of Diabetic Nephropathy more than 80%were overweight or obese.5.The TCM syndromes of 80 patients were all constituted by the main syndromes and the complicated ones.In the main syndromes,qi and yin deficiency type is most common,accounting for 62.5%,followed by the deficiencyof liver-yin and kidney-yin type(16.3%),spleen-kidney-yang deficiency type(12.5%),yin-yang deficiency type(8.8%).In the complicated syndrome,the blood stasis type is accounted for 55%,followed by phlegmatic hygrosis type(25%),damp and hot type(20%).The most common syndrome is qi and yin deficiency with blood stasis type.6.The relationship between the phase Ⅲ diabetic nephropathy’s distribution of TCM main syndromes and the general condition of patients(1)Among the main syndromes,there are no significant difference with sex and(P>0.05).(2)From the BMI,the overweight and obese patients with Qi ard yin deficiency syndrome are most,there are statistical differences(P<0.05).(3)From the course of the disease,yin-yang deficiency type>spleen-kidney-yang deficiency type>deficiency of liver-yin and kidney-yin type>qi and yin deficiency type,there are obvious statistical differences(P<0.05).7.The relatiunship between the phase Ⅲ diabetic nephropathy’s distribution of TCM main syndromes and the correlative indicators(1)There are no signficant difference with HbAlc among the different types of TCM(P>0.05).(2)There are no significant difference with TC、TG、and HDL-C among the diffrent types of TCM(P>0.05);From the LDL-C,yin-yang deficiency type>spleen-kidney-yang deficiency type>liver-yin and kidney-yin type>qi and yin deficiency type.there are obvious statistical differences(P<0.05).(3)From the 24 hours urinary protein,yin-yang deficiency type>spleen-kidney-yang deficiency type>deficiency of liver-yin and kidney-yin type>qi and yin deficiency type(P<0.05).(4)The distribution of TCM main syndromes is related with Scr(P<0.05),but there is no significant difference with UA among the different types of TCM(P>005).(5)From the Cysc.yin yang deficTency type>spleen-kidney-yang deficiency type>qi andyin deficiency type>liver-vin and kidney-yin type,there are obvious statistical differences(P<0.05).Conclusion:1.There was no obvious difference between male and female in the phase Ⅲ of diabetic nephropathy.2.The survey’s analysis result of general information accords with the conclusion that age、overweight or obesity are risk factosr for diabetic nephropathy.3.Qi and yin deficiency with blood stasis type take a primary station among the patients with phase Ⅲof diabetic nephropathy.4.There are differences between the main syndromes of the phase Ⅲ of diabetic nephropathy in BMI、diabetic courses、LDL-C、24 hours urinary protein、Scr and Cysc.It can provide the reference for TCM clinical diagnose of diabetic nephropathy on early stage.
