| Objective:To explore the clinical significance of neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR)and CA 125 in preoperative diagnosis of ovarian endometriosis.Method:Between January 2016 and October 2018,laparoscopic surgery was performed on 149 cases(case group)of ovarian endometriosis and 46 cases of benign ovarian cysts(compare group)in the Department of Obstetrics and Gynecology,Yanbian University Hospital.All cases were included in this study,at the same time 36 healthy women(control group)were selected.Peripheral blood neutrophils,lymphocytes,monocytes and platelet counts were recorded for each group,and NLR,MLR and PLR were calculated.Serum CA125 levels were measured in all patients.Logistic regression was used to assess the clinical significance of NLR,PLR,MLR and CA-125 in ovarian endometriosis.Result:1.The incidence of dysmenorrhea in the case group was higher than that in the control group and the compare group(P<0.05).2.The ratios of NLR and PLR in the case group were higher than those in the compare group and in the control group(P<0.05),CA125 level was higher in case group than in compare group,the median values were 33.8(19.1,62.9)vs.15.7(8.6,34.4)(P<0.05).3.ROC curve results:3.1 ROC AUC 0.867 with 95%CI=0.813-0.922,P<0.001;cutoff value of NLR was 1.95,the sensitivity was 74.5%,the specificity was 86.1%.3.2 ROC AUC 0.682 with 95%CI=0.603-0.761,P<0.001;cutoff value of PLR was 140.51,and the sensitivity was 45.0.%,specificity was 94.4%.3.3 ROC AUC 0.712 with 95%CI=0.609-0.814,P<0.001;cutoff value of CA125 was 18.8,sensitivity was 75.6%,and specificity was 64.7%.4.Multivariate logistic regression analysis showed that high NLR,PLR and CA125 were independent diagnostic markers for ovarian endometriosis.Age>41 years(OR:0.142,95%CI:0.044-0.452,P<0.01)is a protective factor.MPV,MLR and RDW were not significantly associated with the development of ovarian endometriosis.Conclusion:1.NLR,PLR and CA125 are new reference indicators for preoperative diagnosis of ovarian endometriosis.2.Age>41 years is a protective factor for the development of endometriosis,which may be associated with lower levels of estrogen. |
