| Objective : one of the important features of malignancy is inflammation,and chronic inflammatory response is an important factor in tumorigenesis.The aim of this study was to observe and evaluate the neutrophil-lymphocyte ratio(NLR),plateletlymphocyte ratio(PLR)and neutrophil-lymphocyte ratio and platelet product(SII)to provide a reference clinical basis for diagnosis of lung cancer and evaluation of the condition of lung cancer patientMethod:804 patients with lung cancer diagnosed and hospitalized in the First Affiliated Hospital of Dali University from January 2015 to December 2019 and 684 healthy people over 40 years old in the physical examination center of our hospital were included.The factors such as age,gender,smoking history,histological type and TNM stage were statistically analyzed.The differences of some blood cells and NIR,PLR and SII levels in patients with lung cancer,non-small cell lung cancer and small cell lung cancer were also analyzed.The diagnostic value of some blood cells,NIR,PLR and SII in the progression of lung cancer was evaluated.Result:1.In the comparison between lung cancer patients and controls,the values of peripheral white blood cells,monocytes,platelets,NLR,PLR,and SII in the lung cancer group were significantly higher than those in the control group,while the values of neutrophils and lymphocytes were significantly lower than those in the control group(P < 0.05).In the comparison of pathological types in the lung cancer group,neutrophils,lymphocytes,platelets,NLR,PLR,and SII were not statistically significant(P > 0.05).ROC curve analysis of lung cancer showed that the area under the curve of white blood cell,neutrophil,monocyte,platelet,NLR and PLR levels was0.656,0.749,0.700,0.638,0.845 and 0.805,respectively.According to the area under the curve,white blood cell,neutrophil,monocyte,platelet,NLR and PLR had moderate diagnostic value for lung cancer..2.The values of peripheral white blood cells,neutrophils,lymphocytes,monocytes,platelets,NLR,PLR and SII in the lung cancer group were significantly higher than those in the control group(P < 0.05).In the comparison of different clinical stages of NSCLC,the neutrophil level in stage III was significantly higher than that in stage I(P < 0.05);the NLR and PLR level in stage III and IV was significantly higher than that in stage I(P < 0.05);the SII level in stage II,III and IV was significantly higher than that in stage I(P < 0.05).ROC curve analysis of the cell lung cancer group showed that the area under the curve of peripheral white blood cell,neutrophil,monocyte,platelet,NLR and PLR levels was: 0.663,0.751,0.713,0.645,0.845,and0.809,respectively;white blood cell,neutrophil,monocyte,platelet,NLR,and PLR had moderate diagnostic value for non-small cell lung cancer according to the area under the curve.3.The values of peripheral white blood cell,neutrophil,lymphocyte,monocyte,platelet,NLR,PLR and SII in SCLC group were significantly higher than those in control group(P < 0.05).There was no significant difference in white blood cell,neutrophil,lymphocyte,monocyte,platelet,NLR,PLR and SII between different clinical stages of small cell lung cancer(P > 0.05).ROC curve analysis of small cell lung cancer group showed that the area under the curve of peripheral blood neutrophils,lymphocytes,monocytes,and platelets levels was: 0.733,0.193,0.637,and 0.612,respectively;judging the diagnostic value of neutrophils,monocytes,and platelets with low efficacy for small cell lung cancer according to the area under the curve.Conclusion:1.NLR,PLR and SII are inflammatory indicators with clinical disease evaluation value for lung cancer;2.NLR,PLR and SII are inflammatory indicators with clinical disease evaluation value for non-small cell lung cancer,and are correlated with clinical stage of non-small cell lung cancer.3,NLR,PLR and SII are inflammatory markers with clinical disease assessment value for small cell lung cancer,but there is no correlation with clinical stage of small cell lung cancer. |
