The Clinical Efficacy Of TheraCal LC In Deep Dental Caries

Objective:In this experiment,the deep dental caries or reversible pulpitis tooth will be treated by indirect pulp capping with TheraCal LC and calcium hydroxide after step caries removal.In the form of the case report to compare two pulp-cap through the degree of calcification of the dentin and clinical efficacy rate to provide a clinical basis for using TheraCal LC as indirect pulp capping material.Methods:According to the order of visit,60 patients with definite diagnosis of deep caries or reversible pulpitis(60 teeth,all permanent teeth)were randomly divided into TheraCal LC group and calcium hydroxide group,with 30 teeth in each group.Inclusion criteria for cases:1.teeth with definite diagnosis of deep caries or recoverable pulpitis;2.No history of filling or placating therapy,repair therapy or trauma;3.No discoloration,no crack,no wedge defect,no wear,no loose;4.No periodontosis,no pulposis,no periapical disease;5.Pulp activity was normal;6.The patient is in good physical condition,has no long-term history of medication,has informed consent,can cooperate with clinical treatment,and signs the informed consent before treatment.The patients in both groups were treated by step caries removal,and take the X-ray at the first visit.Cleaning the carious tooth tissue around the cavity,removing the thin wall,weak tip and no base glaze by high speed headpieces,removing most of the soft dentine near the pulp by hand instrument.The hardness of residual dentin was examined by probe and the grade was recorded by hardness grading method.Make the teeth dry.In the TheraCal LC group,the pulp was indirectly covered with TheraCal LC,the thickness of TheraCal LC was about 1mm.After Light curing 20 s,the cavity was filled with a glass ionic cement and adjust teeth.In the calcium hydroxide group,the pulp was indirectly covered with calcium hydroxide,After curing,the cavity was filled with a glass ionic cement and adjust teeth.X-ray was taken immediately after treatment andthe thickness and density of dentin were recorded.The possible postoperative symptoms and matters needing attention were asked,and ask the patient returned to the hospital about three months later.X-ray were taken again at the second visit,comparing with one after treatment immediatel to observe the changes in the thickness and density of dentin.Clinical symptoms and signs were examined,and the pulp vitality was tested.The temporary seal and pulp capping materials could be removed for the teeth without obvious clinical symptoms and signs,and the hardness of residual dentin was examined again.Soft caries should be removed again if there are still soft dentin.Make the teeth dry.In the TheraCal LC group,the pulp was indirectly covered with TheraCal LC.The cavity was filled with composite resin,polishing and adjusting teeth.In the calcium hydroxide group,the pulp was indirectly covered with calcium hydroxide,the cavity was based with Poly-F Plus Bondex and filled with composite resin,polishing and adjusting teeth.Inform the patient of the possible complications and matters needing attention after treatment.If discomfort symptoms occur,the patient should return to hospital and treat diaease.The projection positioning still used during the treatmen to ensure the accuracy of measurement of dentin thickness.The hardness level of dentin and the clinical effective rate of each group were recorded,and the results were statistically analyzed.Results:Indirect pulp capping for 3 months,The part of hardness of dentint:TheraCal LC group and calcium hydroxide group had no difference in hardness before treatment,and there was no statistical significance(P>0.05).TheraCal LC group and calcium hydroxide group showed statistica difference in hardness after treatment(P<0.05).After treatment,the percentage of grade 4 hardness(hard)in the TheraCal LC group was significantly higher than that in the calcium hydroxide group,and the percentage of grade 2 hardness(soft)was significantly lower than that in the calcium hydroxide group.The part of the clinical effective rate: TheraCal LC group is 96.7%,calcium hydroxide group is 83.3%,TheraCal LC group and calcium hydroxide group showed statistical difference(P<0.05).Conclusion:Indirect pulp capping for 3 months,TheraCal LC showed better effect of dentin sclerosis than calcium hydroxide,and achieved higher clinical efficiency in indirect pulp capping treatment;TheraCal LC has advantages in clinical application due to its low curing time,no need to prepare and double-layer base.
