Background China has the largest number of hepatitis B patients in the world,and the cumulative five-year incidence of cirrhosis in patients with chronic hepatitis B is 8-20%.The most common direct cause of primary liver cancer is post-hepatitis B cirrhosis.At present,it is believed that the immune function of the body is closely related to the occurrence,evolution and progression of hepatitis B cirrhosis and liver cancer,and the onset and progression of liver disease are usually accompanied by the changes of the immune function of the organism.However,there are few reports about the effects of the changes of cellular immunity and humoral immunity on the development of hepatitis B cirrhosis and liver cirrhosis complicated with liver cancer.Monitoring the results of various indicators of immune function in the body and analyzing their clinical significance have positive effects on evaluating the prognosis of diseases and guiding clinical treatment.Objective To observe the changes of T lymphocyte subsets,immunoglobulins and complement in patients with hepatitis B cirrhosis and liver cirrhosis complicated with liver cancer and their correlation with liver function,and to explore its clinical significance.Methods 50 cases of hepatitis B cirrhosis and 50 cases of liver cirrhosis complicated with liver cancer were selected as the experimental group,and 40 healthy subjects without history of liver disease were selected as the control group.Peripheral fasting venous blood was extracted from 2 groups of patients with chronic liver disease and healthy controls respectively.Cell immunity was detected by flow cytometry:T lymphocyte subsets(CD3~+,CD3~+CD4~+,CD3~+CD8~+,CD3~+CD4~+/CD3~+CD8~+),and the levels of humoral immunity(IgA,IgG,IgM,C3 and C4)were detected by immunoturbidimetry.Statistical methods were used to analyze the differences of the above indicators among the three groups.The correlation between significant indicators(P<0.05)and child-pugh score and MELD score of liver function was discussed through correlation analysis and regression analysis,so as to explore the clinical significance of the above indicators.Results In hepatitis B cirrhosis and liver cirrhosis complicated with liver cancer patients,CD3~+T cells,CD3~+CD4~+T cells and CD3~+CD4~+T cell/CD3~+CD8~+T cells ratio were significantly lower than that of control group,while the CD3~+CD8~+T cells were significantly higher than that of control group.The hepatitis B cirrhosis complicated with liver cancer patients of CD3~+CD4~+T cells was significantly lower than hepatitis B cirrhosis patients,and two groups of CD3~+CD4~+T cells gradually decreased with the rise of liver function score.The content of IgG and IgA in patients with hepatitis B cirrhosis and liver cirrhosis complicated with liver cancer was significantly higher than that in the control group,and there was no significant difference between the two groups,and the serum IgG level in the two groups gradually increased with the increase of liver function score.Serum levels of C3 and C4 in patients with hepatitis B cirrhosis and liver cirrhosis complicated with liver cancer were significantly lower than those in the control group,with no significant difference between the two groups,and serum C4 level in the two groups gradually decreased with the increase of liver function score.Conclusion In patients with hepatitis B cirrhosis and liver cirrhosis complicated with liver cancer,cellular immune function was significantly decreased,serum immunoglobulin level was significantly increased,while complement level was significantly decreased.These indicators were significantly correlated with liver function,but there was no significant difference between the two groups.Clinical detection of the above indicators can not only assess the body’s immune function,but also have important significance for assessing liver function and predicting disease development.In addition,improving the immune function of patients with liver cirrhosis and liver cirrhosis with liver cancer and reconstruction of the body defense system are important therapeutic measures.Monitoring the changes of the body’s immune function has a positive effect on assessing the prognosis of the disease and guiding clinical treatment.
