Purpose:To analyze the effect of arthroscopic tenotomy of the long head of the biceps on the pain of patients with massive rotator cuff tear repair.Method:According to the inclusion-exclusion criteria,a total of 32 patients with rotator cuff tears from October 2016 to February 2018 in our hospital were retrospectively selected and divided into groups A and B:Group A(Arthroscopic repair of massive rotator cuff tear with tenotomy of the long head of biceps)17 people.9 males and 6 females,age 57.27±3.71;group B(arthroscopic repair of the simple rotator cuff tear)15 people.8 males,7 Female patients,age 55.67±3.56,with a follow-up time of 6weeks,3,6 and 12 months after surgery.The VAS(Visual Analog Scale,VAS)score and the Constant-Murley score were used to fully evaluate the degree of pain,function,and range of motion of the shoulder joints before and 6weeks,3,6 and 12 months after surgery.Statistical analysis of the data was performed using SPSS 19.0.Result:After 12 to 18 months of follow-up(mean 14 months),32 patients who underwent MRI after surgery showed good tendon fixation.Patients in group A:VAS preoperative score of 6.53±1.81,postoperative score at 6 weeks 5.40±1.45,postoperative score at 3 months 4.60± 1.30,postoperative score at 6 months 2.87±1.25,postoperative score at 12 months 1.53±1.06;Constant-Murley preoperative score 44.93 ± 5.75,postoperative score at 6 weeks 53.47 ± 5.94,postoperative score at 3 months 57.67±6.20,postoperative score at 6 months 69.93 ± 5.96,postoperative score at 12 months 86.27±2.15.Group B patients:VAS preoperative score of 6.73±1.53,postoperative score at 6 weeks 6.47±1.06,postoperative score at 3 months 6.13±1.19,postoperative score at 6 months 4.13 ± 1.06,postoperative score at 12 months 2.40±0.91;Constant-Murley preoperative score of 44.80 ± 5.23,postoperative score at 6 weeks 46.8±6.19,postoperative score at 3 months 52.07 ± 5.82,postoperative score at 6 months 60.53±5.97,postoperative score at 12 months 70.87±4.66.There were significant differences in functional scores between the two groups at 6 weeks,3 months,6 months,and 12 months(P<0.05).Conclusion:Arthroscopic repair of massive rotator cuff tears with tenotomy of the long head of biceps,and the postoperative shoulder pain and functional recovery were significantly improved in middle-aged and elderly patients.lt is worthy of extensive clinical promotion and use.
