| Background and Purpose:The treatment of massive rotator cuff tears is a challenge for surgeons,and the high rate of retear after surgery is aproblem.Massive,posterior,contracted rotator cuff tears account for a significant proportion of patients.In this category,we attempted to reduce the rate of postoperative retear by using the technique of double rotator interval slide to classify them into repairable types of large rotator cuff tears.In this paper,28 patients with massive,posterior,contracted rotator cuff tearadmitted to the First People’s Hospital of Hefei from January 2019 to August 2020 were retrospectively analyzed to observe the short-term efficacy of rotator cuff space slip technique in arthroscopic repair of posterior massive retractable rotator cuff tear.Methods:Twenty-eight patients with posterior massive retraction rotator cuff tear admitted to our hospital from January 2019 to August 2020 were randomly divided into two groups according to different surgical methods.The control group(14 cases)included 7 males and 7 females,aged 42 to 77(61.57±11.11)years.Anterior rotator interval slide technique was used during the operation.In the observation group(14 cases),6 males and 8 females,aged 43-72(56.64±9.29)years,were treated with anterior and posterior rotator cuff interval slide technique.The operation time and postoperative complications of the two groups were observed,and the improvement of VAS score,Constant-Murley shoulder joint score,Sugaya imaging grade of MRI and Range of motion(ROM)of shoulder joint were compared between the two groups 12 months after surgery.Results:All patients underwent successful surgery and were followed up for 12 months.The mean operation time of the control group was 81.26±5.74 min.There was 1 case of anchor ejection after operation,and no brachial plexus injury or incision infection was observed.VAS score decreased from preoperative(6.71±1.20)to postoperative(2.50±0.85),Constant-Murley shoulder joint score increased from preoperative(28.93±5.53)to postoperative(75.07±7.08),the differences were statistically significant(P < 0.05).The mean operation time of observation group was84.62±7.13 min,and no brachial plexus injury or incision infection was observed.VAS score decreased from 7.21±1.19 before surgery to 1.78±0.80 after surgery.Constant-Murley shoulder joint score increased from preoperative(31.86±5.05)to postoperative(75.93±7.10),with statistical significance(P < 0.05).There was no significant difference in the improvement of Constant-Murley shoulder joint score between the two groups 12 months after surgery(P > 0.05).Twelve months after surgery,the improvement degree of VAS score,active motion of shoulder joint and Sugaya imaging grade of MRI in the observation group were better than those in the control group(P <0.05).Conclusion:The double rotator interval slip technique can significantly improve the VAS score,active forwardflexion motion of shoulder joint and internal and external rotation motion,and the Sugaya imaging grading of MRI after the rotator cuff tear suture,which is worthy of clinical promotion. |
