Objective:To investigate infertile patients treated in Bin Zhou Medical University h ospital’s reproductive medical center,after the pituitary down-regulation,the total gonado tropin use,the oocyte retrieval rate,the quality embryo rate,and the clinical pregnancy rate were adjusted down.This research will give reference to the line of down-regulati on time and the clinical pregnancy rate of.assisted reproductive treatmentMethods:A retrospective analysis of case-assisted in vitro fertilization and embryo transfer in the Reproductive Medical Center of the Bin Zhou Medical University hospit al from January 2016 to December 2016 was conducted in a case-control study and 271 eligible embryo transfer cycles were screened.Patients according to the time of Gn b efore the start of the down regulation was divided into 3 groups:group A,14-15 days(192 cases);group B,16-17 days(54 cases);group C,≥18 days(25 cases).Age,B MI,infertility time,cause of infertility,level of Follicle-stimulating hormone,level of lut einizing hormone,level of basic estradiol,follicle count on the day of HCG,endometri osis thickness,total Gn,Gn days,number of oocytes retrieved,cleavage rate,fertilizatio n rate,high quality blastocyst rate,clinical pregnancy rate,mild to moderate ovarian hy pertrophy OHSS rate,high-quality embryo rate,early miscarriage rate,and ectopic pregn ancy rate were analyzed retrospectively.All patients used a uniform screening standard,a uniform drop standard,and a uniform machine for examination and measurement.Fin ally,all data application software packages were statistically analyzed by chisquare test and statistical description.The test standard P=0.05,P<0.05 was statistically significant.Results:(1)Comparison of patients’ general conditions:The analysis of variance between the three groups were no statistical difference in age,body mass index and infertility age(P>0.05).(2)There was no significant difference in FSH,LH and E2 among the three groups(P>0.05).(3)The total amount of Gn in group A was greater than group B and group C,and the difference was statistically significant(P<0.05).There were significant differences in the endometriosis thickness on the day of HCG injection(P<0.05).(4)There was no significant difference in cleavage rate and blastocyst rate among the three groups(P>0.05).(5)Comparison of clinical outcomes of patients:There was no significant difference in the rate of high quality embryos and clinical pregnancy rate and high-quality embryo rate among the three groups(P>0.05).Conclusions:1.Prolong down regulation days before use of gonadotropins is beneficial for reducing the dose of Gn and the patient’s mental and economic burden,although there are no significant effect on the patient’s pregnancy outcome.2.Although the long-term plan for pituitary down-regulation is the gold protocol currently used,there are still some deficiencies.In clinical applications,it should be based on the patient’s individual differences to adjust the dosage and time.
