| Objective Chronic kidney disease(CKD)is a long-term,life-threatening disease that requires endometrial replacement therapy(including dialysis and kidney transplantation)once it develops into end-stage renal disease.The cost of treatment is high and the quality of life of patients is seriously degraded.Therefore,it is very important to delay the disease progression of patients with chronic kidney disease,prevent serious complications,and improve the quality of life of patients.Traditional Chinese medicine has unique characteristics and advantages in the prevention and treatment of chronic kidney disease.Chinese medicine retention enema treatment is one of the important treatments for chronic kidney disease.Due to its safety and ease of use,it is very common in clinical application.Recent studies have shown that traditional Chinese medicine retention enema can improve the intestinal environment and excrete toxins in patients' intestinal tract.At the same time,it can improve the nutritional status of patients,reduce protein decomposition,reduce the nitrogen-containing toxins in patients,and reduce renal function in CKD patients,delaying their entry into the terminal kidney.Disease,delaying the time of renal replacement therapy[1-4].Patients with chronic kidney disease have poor appetite due to accumulation of toxins,edema of the gastrointestinal tract,etc.,and the energy intake cannot meet the daily demand,and more protein malnutrition is combined,resulting in protein decomposition,which increases the production of nitrogenous waste,thereby aggravating Kidney burden.Therefore,it is necessary to further study the traditional Chinese medicine retention enema combined with oral nutritional supplements to improve the clinical symptoms,biochemical indicators and nutritional status of patients.Methods A total of 54 patients with stage 3 chronic kidney disease who were admitted to the second comprehensive department of Wuhan Central Hospital from December 2016 to December 2018 were randomly divided into the treatment group and the control group,with 27 patients in each group.The patients in the control group were treated with traditional Chinese medicine retention enema(raw rhubarb,calcined keel,calcined oyster,dandelion)in addition to conventional western medicine treatment;the treatment group was supplemented with oral nutritional supplements on the basis of Western medicine and traditional Chinese medicine enema.Nutrison liquid 1.5kcal 500ml(Nutricia(Wuxi)Co.,Ltd.),500 ml per day(1.5kcal per ml,equivalent to 750 kcal per day),taken with meals.14 days was a course of treatment,and the effect was observed after2 courses of continuous treatment.Result After two courses of treatment,the total scores of TCM syndromes of the two groups were lower than before.The total scores of TCM syndromes in the two groups were compared with each other,and the TCM syndrome scores of the treatment group decreased more significantly.In the two groups,the scores of symptoms such as fatigue,weakness of the waist and knees,and heavy limbs decreased,and the decrease of the treatment group was more obvious.The difference between the two groups was P<0.05,and the difference was statistically significant.In the symptoms of nausea and vomiting,low food and numbness,abdominal fullness,sticky mouth and loose stools,the treatment group and the control group decreased compared with before treatment.The symptom scores between the two groups were P>0.05,and the difference was not statistically significant difference.The effective rate of the treatment group was 48.14%,while the effective rate of the control group was 37.03%.The serum creatinine,urea nitrogen and e GFR of the treatment group and the control group were lower than before treatment.The albumin and prealbumin in the treatment group were higher than before,and the prealbumin in the control group was higher than before.P<0.05 was statistically significant difference.There was no significant difference in albumin between the control group and before treatment.There was no significant change in hemoglobin treatment before and after treatment in the treatment group and the control group.Compared with the control group,the treatment group had lower urea nitrogen,higher albumin and prealbumin,and P<0.05 was statistically different.Conclusion The addition of traditional Chinese medicine retention enema combined with oral nutritional supplements on the basis of conventional Western medicine treatment can not only significantly improve the clinical symptoms of patients,but also improve the nutritional status of patients,reduce the patient's condition,and improve the overall quality of life of patients. |
