The Retrospective Study On Treatment Of Chronic Kidney Disease At Stage 3-5 With Compound Rhubarb Enema

ObjectiveWith analyzation of the alleviation of symptoms observed in traditional Chinese medical way and biochemical indicators of patients of chronic kidney disease(CKD)at stage 3-5 without dialysis,we evaluated the clinical efficacy and safety of compound rhubarb enema in the treatment of CKD at the same stage,aiming to give reference to improve living quality of patients of CKD and suspend the deterioration of CKD,which can give full play to the features and advantages of Traditional Chinese Medicine in the treatment of CKD.MethodsRetrospective study were applied on hospitalized cases collected from May 2018 to December 2018 in the division of nephrosis of the first affiliated hospital of Guangzhou University of Traditional Chinese Medicine.We divided the 400 hospitalized cases met our selection criteria into the observation group and the control group.200 cases in the control group were treated with western medicine while the other 200 cases in the observation group were treated with western medicine combined preserving clyster using compound rhubarb enema.Following that,we collected the general data of patients,therapeutic effect index such as serum creatinine,blood urea nitrogen,glomerular filtration rate,hemoglobin and scores of TCM symptoms as well as safety index like potassium,calcium,sodium,AG and TC02 before and after treatment.SPSS20.0 was used for statistical analysis of the data to analyze the clinical efficacy and safety of compound rhubarb enema in the treatment of chronic kidney disease at stage 3-5.Results(1)The total effective rate of the observation group was 71.5%;The total effective rate of the control group was 61.5%,and the difference between the two groups were statistically significant(P<0.05).(2)The scores of TCM symptoms in the two groups after treatment were significantly lower than those before treatment(P<0.05).After treatment,scores of TCM symptoms in the observation group decreased significantly compared with the control group(P<0.05).(3)Compared with that before treatment,the improvement of serum creatinine,blood urea nitrogen and glomerular filtration rate in the two groups after treatment was significant,and the difference was statistically significant(P<0.05).There was no significant difference in hemoglobin between the two groups after treatment(P>0.05).After treatment,the decrease of serum creatinine and blood urea nitrogen in the observation group was more significant than that in the control group(P<0.05),and the increase of glomerular filtration rate in the observation group was more significant than that in the control group(P<0.05).(4)The total effective rate of CKD at stage 3 in the observation group was 87.9%;the total effective rate of CKD at stage 4 in the observation group was 75%;the total effective rate of CKD at stage 5 in the observation group was 40.5%.The difference was statistically significant(P<0.01).(5)After treatment,potassium,sodium,calcium and AG in the control group and the observation group all decreased compared with that before treatment,but the differences in potassium,sodium and calcium levels before and after treatment were not statistically significant(P>0.05),and there were significant differences between AG in two groups after treatment and that before treatment(P<0.05).However,there were no statistically significant differences in AG between the two groups after treatment(P>0.05).After treatment,TC02 in both control group and observation group increased compared with that before treatment,but there was no significant difference in TC02 between the two groups after treatment(P>0.05).Conclusion(1)Preserving clyster using compound rhubarb enema to treat patients of CKD at stage 3-5 not only can reduce the scores of TCM symptoms,alleviate the clinical symptoms,improve the living quality of patients,but also can reduce serum creatinine,blood urea nitrogen and increase glomerular filtration rate of CKD at stage 3-4,which can protect residual renal function and suspend the deterioration of CKD.In addition,preserving clyster using compound rhubarb enema is with high security.(2)Preserving clyster using compound rhubarb enema for CKD at stage 3 had the best effect,followed by CKD at stage 4 and there was nearly no effect in CKD at stage 5,indicating that the worse the renal function was,the curative effect of preserving clyster using compound rhubarb enema was gradually decreased.
