Clnical Analysis Of Nasal Of Nasal Endoscopic Dacryocystorhinostomy For Different Types Of Chronic Dacryocystitis

Objective:To classify the lacrimal sac cavity according to the size and lesion of the lacrimal cavity in dacryocystorhinostomy under nasal endoscopic,and observe the curative effect of different types of chronic dacryocystitis.Method:Nasal endoscopic dacryocystorhinostomy was performed in 56 patients(60 eyes)with chronic dacryocystitis.According to the condition of the lacrimal cavity,they were divided into normal or large dacryocystoric cavity chronic dacryocystitis(group A),Chronic dacryocystitis with the small lacrimal cavity(group B)and chronic dacryocystitis with the dacryocyst granulation adhesion group(group C);During regular postsurgery follow-up,the observation and comparison the efficacy of three types of chronic dacryocystitis in 1 month and 4 months after surgery were carried out,at the same time,the genesis of different types of lacrimal sac was analyzed.Result: A total of 56 patients(60 eyes)were collected.In the past history,the lacrimal duct intubation,dacryocystorhinostomy(including traditional approach and nasal endoscopic approach),laser lacrimal ductplasty,and lacrimal duct high frequency electrocautery,the other treatments were 20 eyes,6 eyes,4 eyes,6 eyes,and 29 eyes.The difference between the previous treatment history of the three groups was statistically significant(P<0.05),There was significant difference between group A and group B and group A and group C(P<0.05),but there was no significant difference between group B and group C(P>0.05).(Note: Excluding cases of dacryocystorhinostomy).The total recovery rate was 93.3% and the total effective power was 98.3% in all cases.The total recovery rate was 81.7% and the total effective power was 90%.Among them,39 cases of 39 eyes in group A,the recovery rate was 97.4% in 1 month postsurgery,the effective power was 100%,the recovery rate was 94.9% in 4 months after operation,the effective power was 97.4%;11 cases in group B,14 eyes,The recovery rate was 92.9% and the effective power was 100% at 1 month after operation.The recovery rate was 64.3% and the effective power was 85.7% at 4 months after operation.7 cases of 7 eyes in group C,the recovery rate was 71.4% in 1 month postsurgery.The effective power was 85.7%,the recovery rate was 42.9% in 4 months postsurgery,and the effective power was 57.1%.There was no significant difference in the effective power between the three groups(P>0.05).There were significant differences between the three groups at 4 months after operation(P<0.05),and there were significant differences between the A and C groups(P<0.05),but no differences between the B and C groups and the A and B groups(P>0.05),there was no statistically significant difference in the effective power of 1 month and 4 months after surgery in all patients and groups.Conclution:Nasal endoscopic dacryocystorhinotomy is satisfactory.Different types of chronic dacryocystitis have different curative effects,it is of great value to judge the postsurgery efficacy and guide the follow-up treatment according to the condition of the lacrimal sac in the operation.
