| ObjectiveTo investigate the early differentiations of necrotizing pneumonia(NP)and congenital pulmonary cyst complicated with infection(CPCI)in children through comparison of changes in laboratory related inspection indicators and chest CT scans,and provide clinical reference for the selection of appropriate treatment regimen.Methods39 patients with NP or CPCI who were admitted to Department Pediatrics of the First Affiliated Hospital of Zhengzhou University from August 2014 to March2019,and the course of disease is less than 10 days were selected as the subjects of the study.According to the types of diagnosed disease,it is divided into necrotizing pneumonia group and congenital pulmonary cyst complicated with infection group.Among them,21 cases of necrotizing pneumonia were recorded as Group A,and 18cases of congenital pulmonary cyst complicated with infection were recorded as Group B(Hereinafter referred to as Group A and Group B).After admission to the hospital,gauge blood routine,C reactive protein(CRP),erythrocyte sedimentation rate(ESR),lactate dehydrogenase(LDH),procalcitonin(PCT),D-dimer,Serum ferritin(SF)and chest CT scans and other related examination indicators.Compare the changes of related indexes in two groups of children to conclusion.The data is processed using SPSS 22.0 statistical software.The counting data is subject to two independent sample T tests for normal distribution,and is represented by?x±s.If not,using M(Q25,Q75)spacing.The ROC curve is plotted for indicators with statistical differences.There are statistical differences when P<0.05,p<0.01 represents the differences are Significant.Results1.The levels of C-reactive protein,procalcitonin,erythrocyte sedimentation rate,D-dimer,lactate dehydrogenase and ferritin in children with necrotizing pneumonia were higher than those in patients with congenital pulmonary cyst complicated with infection(P<0.05).The levels of C-reactive protein,procalcitonin,D-dimer,lactate dehydrogenase and ferritin in necrotizing pneumonia group were significantly higher than those in congenital pulmonary cyst complicated with infection group(P<0.01).2.The two groups of diseases were characterized by a slightly high-density speck,with one or more caplets.And a gas-liquid plane is likely present in the cavity of the congenital pulmonary cyst complicated with infection.Conclusion1.In chest imaging,the presence of gas-liquid plane in single or multiple cystic cavities with flake or flocs of high density suggests of congenital pulmonary cysts complicated with infection.2.When the imaging findings of bronchopneumonia are solid or empty,The high levels of D-dimer,lactate dehydrogenase and ferritin in peripheral blood should be highly vigilant against the occurrence of necrotizing pneumonia. |
