Objective:The clinical application of Compound Panax Japonicus tablets combined with methotrexate tablets and chloroquine tablets in the treatment of RA,to observe the main symptoms and signs,related laboratory indicators and serum levels of TAK-1,JNK,MMP-3 before and after treatment,To explore the clinical efficacy and possible mechanism of action of the drug.The AA rat model was established and treated with Compound Panax Japonicus tablets.The therapeutic effect of the drug on AA rats and its possible mechanism were investigated by detecting the expression of TAK-1,JNK and MMP-3 proteins in the synovium of knee joint.Provide a theoretical basis for further clinical application of the drug.Methods:Clinical study: 1 60 patients with RA who were treated in the outpatient department and hospitalized in the Chinese medicine department of Minda Hospital affiliated to Hubei University for Nationalities from May 2018 to January 2019 were randomly divided into the control group and the observation group,30 cases in each group.Oral methotrexate tablets and chloroquine tablets were given.The observation group received oral Compound Panax Japonicus tablets on the basis of the control group.The two groups were given intermittent medication for 8 weeks.2 The patients were observed and recorded before treatment and after 8 weeks of treatment.Clinical symptoms and signs,including joint tenderness,joint swelling,VAS score and major syndrome scores;detection of ESR,RF and liver and kidney function(mainly ALT,AST,SCr,BUN);ELISA test for serum TAK-1,JNK,MMP-3 content.Animal experiments: 1 60 male Sprague-Dawley rats(body weight 200±20g)were randomly divided into normal group(15 rats)and model group(45 rats).Except the normal group,the other AA models were constructed.After successful modeling,They were randomly divided into model group,methotrexate group and Compound Panax Japonicus group,with 15 rats in each group.After 1 week of immunization,each group was given the corresponding drug dose,and the normal group and the model group were given the same amount of normal saline for intragastric administration,and were sacrificed after 7 weeks of continuous administration.2Observe and record the general condition of the rats,including the weight of the rats,the swelling of the feet,the arthritis index,etc.;detect the levels of ALT,AST,SCr and BUN in the serum of rats;and detect the synovial membrane of the knee joints in rats by Western Blot The content of TAK-1,JNK and MMP-3 protein changes.Results:Clinical study: 1 There were no significant differences in joint tenderness,joint swelling,VAS score,ESR and RF between the two groups before treatment(P>0.05).After treatment,the number of joint tenderness in the observation group compared with the control group.The number of joint swelling,VAS score,ESR and RF were improved(P<0.05).2 There was no significant difference in the main symptoms and signs between the two groups before treatment(P>0.05).The main symptoms and signs of the two groups after treatment The scores were lower than those before treatment(P<0.05).The observation group improved significantly compared with the control group(P<0.05).3The serum ALT,AST,SCr and BUN did not change significantly before treatment(P>0.05);In addition,the serum levels of ALT,AST and SCr in the two groups were higher than those before treatment(P<0.05).After treatment,the levels of TAK-1,JNK and MMP-3 in the serum of the two groups were lower than those before treatment(P<0.05).The observation group was significantly lower than the control group(P<0.05).Animal experiment: 1 General situation: Compared with the normal group,the model group lost weight,paw swelling,and arthritis index increased significantly.Compared with the model group,the two groups had increased body weight,paw swelling,and arthritis index decreased.However,the compound bamboo ginseng group had a slower weight gain than the methotrexate group(P<0.05),and the paw swelling and arthritis index decreased slowly(P<0.05).2 Compared with the model group,the methotrexate group The serum levels of ALT,AST,SCr and BUN in the Compound Panax Japonicus group were increased(P<0.05),and the methotrexate group was significantly increased(P<0.05).3Western Blot method: compared with the normal group Compared with the model group,the Compound Panax Japonicus tablets group and the methotrexate group,the expression of TAK-1,JNK and MMP-3 protein in the synovium of the knee joint was increased(P<0.05);compared with the model group,the compound The expressions of TAK-1,JNK and MMP-3 in the synovium of the knee joint of the Compound Panax Japonicus group and the methotrexate group were decreased(P<0.05),but the Compound Panax Japonicus tablets group was slightly higher than methotrexate.Group(p<0.05).Conclusion:1 Compound Panax Japonicus tablets have a good therapeutic effect on AA model rats.2 Compound Panax Japonicus tablets combined with western medicine treatment is superior to western medicine group in relieving joint pain and joint swelling and other clinical manifestations and laboratory indicators.3 The mechanism of compound bamboo ginseng tablets in the treatment of RA may reduce the expression of TAK-1,JNK and MMP-3 in serum and synovial membrane,and inhibit the activation of JNK signaling pathway to relieve cartilage and bone destruction.
