The Relationship Between Subclinical Hypothyroidism Or Peroxidase Antibody And Preterm Birth During Pregnancy:A Meta-analysis

Objectives:Whether subclinical hypothyroidism(SCH)and thyroid peroxidase antibody(TPOAb)increase the probability of preterm birth during pregnancy,and there is still a large inconsistency in the need for treatment for these people.Meta-analysis was used to systematically evaluate the correlation between pre-and post-natal treatment of SCH and TPOAb in pregnancy,and to explore whether there is a relationship between them in order to obtain more accurate and objective conclusions,in order to provide more effective,reliable clinical diagnosis and treatment? Methods:Data of PubMed?Medline?Embase?Web of Science?Cochrane Library?CBM?CNKI?Wan Fang etc.According to the established literature inclusion andexclusion criteria,find and incorporate statistical analysis.Search time limitas of February 2019,two researchers independently screening,quality evaluation and data extraction,conversion,evaluation of evidence quality using GRADE software,Statistical analysis of datausing RevMan5.3 software,combined OR and its 95% confidence interval.Main results:1.A total of 18 studies were included,including 11 in English and 7 in Chinese.NOS scores were ? 5 points,including 25,106 participants;2.The meta-analysis results of this study: the incidence of preterm birth in pregnant women with TPOAb(+),SCH group,TPOAb(-),SCH group and TPOAb-positive group was significantly higher than that of normal pregnant women.However,after drug treatment,TPOAb alone The incidence of premature birth in pregnant women with positive,TPOAb-positive and subclinical hypothyroidism decreased,but for patients with TPOAb-negative and subclinical hypothyroidism,drug treatment did not significantly improve the prevention of preterm birth.Conclusion:Pregnant women with SCH and simple TPOAb positive are risk factors for the lack of monthly delivery in women during the reproductive period,especially in pregnant women with TPOAb positive and SCH and TPOAb alone.The incidence of under-delivery is higher.The treatment of Youjiale can reduce the risk of preterm birth in TPOAb-positive SCH pregnant women and TPOAb-positive pregnant women.For patients withTPOAb-negative SCH,clinical trials show that drug treatment has no ideal effect.
