| Background Endometrial lesions are high-risk gynecological diseases.In recent years,the incidence has been growing year by year,Among the more common lesions: endometriosis and endometriosis are not obvious in the change of endometriosis thickness,endometriosis polyps are more limited,ultrasound two-dimensional images have characteristics,and the diagnostic rate is higher;Ultrasonic diagnosis of endometrial hyperplasia and endometrial carcinoma is the focus of this study.Early endometrial carcinoma(EC)and endometrial hyperplasia(EH)are two common types of endometrial lesions.They have similar early clinical symptoms,mostly including irregular vaginal bleeding,discharge,menstrual disorder,etc.Two-dimensional ultrasound often shows the regular and irregular thickening of the endometrium,and endometrial lesions are shown as color blood flow signals in Color Doppler Flow Imaging(CDFI).It is often difficult to distinguish the two diseases based on the preliminary examinations,which may delay the optimal treatment time and causes great harm to women mentally and physically.At present,the diagnosis of endometrial cancer in China mainly relies on histopathological examination.However,given the relatively traumatic collection of endometrial tissue in contrast to ultrasound,a repeatable and non-invasive examination method,it is imperative to look for a meaningful ultrasonic differential diagnosis method.By studying the blood flow parameters of thickened endometrium in Transvaginal Color Doppler(TVS)including Resistance Index(RI)and Pulsatility Index(PI),microvessel density(MVD)of thickened endometrium,the paper explores the significance of differential diagnosis of endometrial hyperplasia and endometrial cancer.Objectives To observe the color blood flow parameters of TVS(RI and PI)and the microvessel density(MVD)of tissue section and analyzed their difference and relationship in endometrial hyperplasia and endometrial cancer,and changes in different clinical stages of endometrial cancer.To explore the value of blood flow parameters of TVS and MVD in the differential diagnosis of endometrial lesions,as well as the significance of identifying and prognosis of endometrial cancer.Method The endometrial cancer group consisted of 69 patients with endometrial cancer diagnosed through histopathology and enrolled in the hospital from January 2015 to October 2017,and 52 patients with endometrial hyperplasia who were diagnosed at the same period were selected as control group.According to standards of the International Federation of Gynecology and Obstetrics in 2009,the endometrial cancer group was divided into Phase I group,Phase II group,and Phase III group.TVS blood flow parameters(RI and PI)?and endometrial tissue MVD levels were collected from the endometrial cancer group and the endometrial hyperplasia group respectively.Data were analyzed by SPSS22.0 statistical software to compare the RI,PI and MVD levels were compared between the endometrial hyperplasia group,the endometrial cancer group and its FIGO groups of different phases,and explores the relationship and significance of blood flow parameters(RI and PI)and MVD levels.Result:1.The endometrial cancer group showed more blood flow signals than the control group.2.Compared with the control group,the TVS blood parameters RI and PI of the endometrial cancer group were lower and the difference had statistical significance.3.Compared with the control group,the MVD levels of the endometrial cancer group were higher and the difference had statistical significance(P<0.05).4.The endometrial cancer group was divided into FIGO phase I,II,III,with the RI and PI of the TVS gradually decreasing,while the MVD levels gradually rising.The differences between the groups all had statistical significance(P<0.05).5.There was a negative correlation between RI,PI and MVD levels in patients with endometrial cancer(r =-0.677,-0.683,P < 0.05).Conclusion:1.The blood flow parameters RI and PI of TVS have clinical significance for differential diagnosis of endometrial hyperplasia and endometrial cancer.2.The MVD levels in endometrial tissue can provide a basis for the diagnosis of endometrial cancer.3.The blood flow parameters RI and PI,and MVD level are negatively correlated,which can further reflect the MVD level to evaluate the formation of microvessels in the endometrium.4.The PI,RI and MVD level of patients with different clinical stages of endometrial cancer show certain patterns of changes,The microangiogenesis and blood flow parameters of endometrial carcinoma can provide a basis for clinical staging,development and even prognosis evaluation of endometrial carcinoma. |
