Objective(1)To investigate the positive rate of HBsAg and HBsAb in the preschool children in Jiangnan district of Nanning,to evaluate the effect of immunization of hepatitis b vaccine in preschool children,and to provide guidance for the prevention and control of hepatitis B in our city.(2)At end of cohort study,the positive rate of HBsAg compared with hepatitis b vaccine non-responders and hepatitis b vaccine responders.To explore the hepatitis b vaccine non-responders are HBV susceptibles or resistants,and to evaluate the risk of HBV infection in the non-responders to hepatitis b vaccine in future.Methods(1)During the period from March 2015 to June 2016,the blood of preschool children from 82 kindergartens were collected in Jiangnan district of Nanning,HBsAg and HBsAb were detected by ELISA assay.(2)The prospective cohort study was used in our research.The children aged 2 to 5 years and from 64 kindergartens were selected in our study.These children were inoculated three doses of hepatitis b vaccine by 0,1 and 6 months.The HBsAg and HBsAb were tested during the period from March to May 2015.Excluding HBsAg positive children,the subjectes were divided into exposure group(HBsAb negative)and control group(HBsAb positive)according to the HBsAb positive or negative.The study was followed up for one year,beginning on June 1,2015,and the end on July 1,2016.On June 1 to 30,2016,the serum HBsAg of children was detected in the cohort.At the end of the study,there was a comparison of HBsAg positive rate between the non-responsive group and the response group.Results(1)The results of the cross-sectional study :10757 preschool children(3-6 years old)from 82 kindergartens were tested totally.A total of 24 people were HBsAg positive and the total positive rate of HBsAg was 0.22%,there was no difference in HBsAg positive rate between different ages and genders.The number of HBsAb positive children was 6195 and the total positive rate of HBsAb was 57.59%.There was a difference in the positive rate of HBsAb for different ages.The HBsAb positive rates of children aged 3,4,5 and 6 were 51.96%,55.02%,60.79%,65.71%,respectively.The positive rate of HBsAb in children increased with age.There was no difference in HBsAb positive rate between different genders.(2)The cohort study results : The 1824 non-respondent were follow-uped and 5 subjects were found to be HBsAg positive.The positive rate of HBsAg in the non-response group was 0.27%.The 2054 responders were observed in the control group and none of them were found to be positivity for HBsAg,and so the HBsAg positive rate was 0.00%.There was a significant difference in the positive rate of HBsAg between the non-response group and the response group for one year(p <0.05).Conclusion(1)The positive rate of HBsAg in preschool children is only 0.22% in Nanning,but the positive rate of HBsAb is not high(51.96%-65.71%).(2)The HBsAg positive rate in the non-response group was 0.27%,and it was higher than the response group(0.00%).Therefore,the non-responders of hepatitis B vaccine still face the risk of HBV infection.
