ObjectiveMultiple myeloma is a hematological malignant tumor characterized by abnormal proliferation of plasma cells in the bone marrow.It is the second most common malignant disease in the blood system,and its incidence is second only to lymphoma.10%of hematopoietic tumors account for 1%of all cancer deaths,and the incidence of the tumors is increasing.The patients diagnosed need to be admitted to the hospital for examination,review,treatment,etc.Since most of the patients are elderly,the prognosis of patients is poor.So far,multiple myeloma is still an incurable disease,which seriously endangers human health.At present,the prolonged survival period is the main therapeutic purpose,and the relationship between clinically relevant indicators and prognosis is compared to find out the survival of patients.Prognostic factors;improve risk of life and prolong survival by controlling risk factors.MethodAll eligible patients with multiple myeloma admitted to a top three hospital in Shenyang from January 1,2011 to December 31,2015 were selected.Collect basic information,laboratory results,hospitalization expenses and other information through the hospital’s medical record system.The data was analyzed using SPSS 22.0 software.Continuous variables were described by the mean and standard deviation of the normal distribution.And data that don’t obey the normal distribution were described by the median and interquartile range.The categorical variables were described by frequency.The Kaplan-Meier method were used for calculate cumulative survival rate,the survival period was compared by Log-rank test,and the Cox proportional hazard regression model was used for multivariate analysis.The test level is set to 0.05.ResultA total of 259 patients with multiple myeloma were included in the study.As of June 30,2017,132 patients died,110 patients survived,and 17 patients were lost to follow-up.The rate of loss of follow-up was 6.56%.The median age is 60 years old.There were 151 males and 108 females,the ratio of male to female was 1.40:1;the largest number of IgG was 111,accounting for 42.86%.The median survival of patients with multiple myeloma was 46 months,with a minimum of 1 month and a maximum of 153 months.The two-year survival rates were 84.8%,the three-year survival rates were 61.2%,and the five-year survival rates were 36.5%.Univariate analysis showed clinical classification,International Staging System,Durie-Salmon Staging System,treatment plan,hemoglobin,white blood cell count,neutrophil count,serum albumin,serum calcium ion,serum potassium ion There was no significant difference in creatinine level(P>0.05).Age,gender,complications,course of treatment,platelet count,red blood cell count,serum β2 microglobulin,urinary tract β2 microglobulin,kappa light chain level,lamma light chain level were statistically significant(P<0.05).Multivariate analysis showed that gender,complications,red blood cell count,urinary tract β2 microglobulin,and lamma light chain levels were not statistically significant(P>0.05).There were significant differences in age,course of treatment,kappa light chain,serum β2 microglobulin and platelet count(P<0.05).ConclusionMultiple myeloma occurs in middle-aged and elderly men with a median survival of 46 months,with a median survival of 53 months in the new drug group and 45 months in both the traditional and mixed groups.The two-year survival rates were 84.80%,the three-year survival rates were 61.20%,and the five-year survival rates were 36.50%.Age,kappa light chain level,serum β2 microglobulin,platelet count,and course of treatment are independent prognostic factors for patients with multiple myeloma.Age<55 years old,platelet count≤100×109/L,serum β2 microglobulin>3.5 mg/L,kappa light chain<3.7g/L,treatmen<4 is an independent prognostic factor for patients with multiple myeloma.
