ObjectiveStudies have shown that epicardial adipose tissue is associated with the occurrence and maintenance of atrial fibrillation.This study aimed to investigate the relationship between atrial fibrillation and atrial fibrillation by measuring sinus heart rate and epicardial adipose tissue volume in patients with atrial fibrillation.MethodsIn this study,66 patients with atrial fibrillation who received left atrial CTA due to catheter ablation were enrolled in the cardiovascular center of the first hospital of jilin university from August 2018 to February 2019,including 31 patients with persistent atrial fibrillation(47%)and 35 patients with paroxysmal atrial fibrillation(53%).Jilin university first hospital outpatient service line with same period 256 row of Coronary CT Angiography(CCTA)out of Coronary heart disease and 50 cases of sinus rhythm of atrial fibrillation group is not af group as the control group.1.All patients were recorded for their age,gender,BMI,type of atrial fibrillation,course of atrial fibrillation,CHA2DS2-VASc score,history of diabetes,history of hypertension,blood routine,biochemical,blood lipid and other indicators..2.Patients with atrial fibrillation underwent routine left atrial CTA and esophageal ultrasonography before atrial fibrillation ablation.The control group was sinus rhythm group.The heart rate was controlled at 60-75 beats/min,and the results showed no significant stenosis in the coronary arteries.The original data was uploaded to the Philips workstation for 3D imaging and multi-planar recombination and other 3D processing.Using the Volume software analysis,the epicardial adipose tissue volume was calculated.3.All patients used the human body composition analyzer [Instrument model 5157021159(seca515)] for human body composition analysis,and calculated the fat weight,body fat rate,visceral fat,waist circumference and other indicators of the reflected body fat level.Results:1.BMI,hypertension,left atrial diameter,BNP,D-D and uric acid of the patients in the atrial fibrillation group were significantly higher than those in the sinus rhythm group,and LVEF(%)was lower than those in the sinus rhythm group,with statistically significant differences(P < 0.001).2.The fat weight of patients in the atrial fibrillation group [(25.44±5.09)Kg vs(21.16±4.60)Kg,P < 0.001] was higher than that in the sinus rhythm group.The body fat rate [(36.17±8.79)% vs(32.54±6.32)%,P=0.015] in the atrial fibrillation group was higher than that in the sinus rhythm group.Visceral fat in patients with atrial fibrillation [(3.18±0.94)Kg vs(2.81±1.01)Kg,P=0.045] was higher than that in sinus rhythm group.LDL-C [(2.68 ± 0.84)mmol/L vs(2.31 ± 0.70)mmol/L,P=0.009] was higher in patients with atrial fibrillation than in sinus rhythm group.3.Epicardial adipose tissue volume: epicardial adipose tissue in patients with atrial fibrillation [(135.42±12.68)ml vs(113.50±13.41)ml,P<0.001] was significantly greater than sinus rhythm group;epicardial adipose tissue in patients with persistent atrial fibrillation Volume [(138.81 ± 13.40)ml vs(132.43 ± 11.37)ml,P = 0.04] was greater than the group of patients with paroxysmal atrial fibrillation.4.Independent risk factors for atrial fibrillation: Multivariate logistic regression analysis showed: epicardial adipose tissue volume(OR=1.087,95% CI 1.026-1.152,P=0.005),left atrial diameter(OR=1.292,95%CI 1.082-1.543,P=0.005)is an independent risk factor for atrial fibrillation.5.ROC curve: epicardial adipose tissue volume(EATV): The area under the curve is 0.884.When the cutoff value is 122.0 ml,the sensitivity and specificity of predicting atrial fibrillation are 87.9% and 74.0%,respectively;The area under the curve was 0.898.When the cutoff value was 33.5 mm,the sensitivity and specificity of predicting atrial fibrillation were 84.8% and 84.0%,respectively.Conclusions:1.The fat weight,body fat percentage,visceral fat,and low-density lipoprotein cholesterol in the atrial fibrillation group were larger than those in the sinus rhythm group.The difference between the two groups was statistically significant.2.The volume of epicardial adipose tissue in patients with atrial fibrillation was significantly larger than that in sinus rhythm group,and the volume of epicardial adipose tissue in patients with persistent atrial fibrillation was larger than that in patients with paroxysmal atrial fibrillation group.3.Left atrial diameter and epicardial adipose tissue volume are independent risk factors for atrial fibrillation and have a good predictor of atrial fibrillation.
