The Correlation Of Left Atrial Epicardial Adipose Tissue CT Density And Atrial Fibrillation

Objective:To evaluate the association between multi slice spiral CT-derived left atrial epicardial adipose tissue(LA-EAT)CT density and atrial fibrillation(AF).Methods:44 patients of paroxysmal AF and 31 patients of persistent AF in Shengjing Hospital of China Medical University from March January 2018 to April 2020were Enrolled,and 50 cases of sinus rhythm were chosen as control group.256 slice spiral CT was applied in all these 125 patients.LA-EAT CT density and epicardial adipose tissue volume(EATV)in all patients were measured,and general data was collected.LA-EAT CT density,EATV and related data were compared in patients with paroxysmal AF group,persistent AF group and sinus rhythm group.Logistic regression analysis was used to analyze whether LA-EAT CT density is an independent risk factor for the presence of AF.The receiver-operating characteristic(ROC)curve of LA-EAT CT density and other indicators were drawn to predict the incidence of AF.Results:The LA-EAT CT density was significantly larger in paroxysmal AF and persistent AF patients compared with that of sinus rhythm group,[(-109.9±5.9)HU vs(114.6±4.1)HU,P<0.001];(-110.5±7.0)HU vs(-114.6±4.1)HU,P<0.001].There was no significant difference in LA-EAT CT density between persistent AF group and paroxysmal AF group.LA-EAT CT density,EATV,and left atrial diameter(LAD)are independent risk factors of AF(OR=1.273,95%CI 1.139～1.423,P<0.01;OR=1.033,95% CI 1.017～1.141,P<0.01;OR=1.077,95%CI 1.017～1.141,P<0.05).The area under the curve(AUC)of LA-EAT CT density,EATV,LAD to predict the presence of AF were 0.740,0.724,0.750,respectively,the optimal thresholds are 110.7 HU,110.3cm~3,37.5mm,and the AUC of the combined three prediction of atrial fibrillation is0.861.Conclusion:The increase of LA-EAT CT density is significantly correlated with the presence of AF,However,there was no statistically significant difference in LA-EAT CT density between different types of atrial fibrillation.EAT is one of the independent risk factors for the presence of AF.LA-EAT CT density has a certain predictive value for the occurrence of atrial fibrillation,and its combination with EATV and LAD can increase the predictive value of atrial fibrillation.
