Objectives To investigate the knowledge and influence factors of hepatitis B in the soldiers and provide scientific basis for the army to formulate health education measures and preventive measures for hepatitis B.To study the immune effect of different vaccination regimens of hepatitis B vaccine in soldiers and to explore the immunization regimen of hepatitis B vaccine,which are more suitable for the actual situation ofsoldiers.Methods A total of 359 new soldiers enrolled in a military college in September 2017 were screened for negative serum hepatitis B surface antibody and hepatitis B surface antigen by ELISA.A modified hepatitis B knowledge questionnaire was used to investigate the general information and basic knowledge of hepatitis B in 326 recruits.According to the inclusion criteria and exclusion criteria of hepatitis B vaccination,338 soldiers were included and the enrolled subjects were divided into four groups by using cluster sampling method.Traditional group(n=71):20 μg vaccine at 0,1,6 months;Accelerated group(n=99): =20 μg vaccine at 0,1,2 months;Traditional booster group(n=77):40 μg vaccine at 0,1,6 months;Accelerated booster group(n=91):40 μg at 0,1 and 2 months,respectively.2 ml peripheral venous blood was collected on the 30 th day after each vaccination and the serum anti-HBs titer was detected by ELISA qualitative method.The positive conversion rate,time efficiency and safety of anti-HBs were compared among the four regimens.Results The general Awareness Rate of hepatitis B knowledge was 74.25%.The Awareness Rate of hepatitis B knowledge was different in different regions,and the Awareness Rate of hepatitis B knowledge was higher in urban areas than that in rural areas(t=10.79,P<0.001).The Awareness Rate of hepatitis B knowledge was higher among the new soldiers with college or above education degree and than that high school or below education degree,the difference had statistical significance(t=8.45,P=0.031).The network is the most important way to know the knowledge of hepatitis B(88.04%).The positive conversion rates of anti-HBs in peripheral blood were 90.14%,90.91%,98.70% and 98.90% in the traditional group,accelerated group,traditional enhanced group and accelerated enhanced group,respectively.At the same dose,there was no statistical difference in the positive conversion rate between 0,1,2 months regimen and 0,1,6 months regimen(P> 0.05),but the vaccination period was significantly shortened.Under the same inoculation time point,the positive conversion rate of 40 μg/ inoculation was significantly higher than that of 20 μg/ inoculation(P< 0.05)and there was no significant difference in the incidence of adverse reactions(X~2=0.11,P=0.744).Conclusions The Awareness Rate of hepatitis B knowledge in different areas and different education background is different.For the new soldiers from rural areas with low education background,it is necessary to strengthen the education of hepatitis B knowledge.Increasing the appropriate dose significantly increased the success rate of hepatitis B vaccination in soldiers with good safety.Hepatitis B vaccine has a short cycle and high success rate,which is suitable for popularization and application in the new soldiers.
