Objectives:1.To study the characteristics of muscle function and quality of life of the elderly in different genders and ages.2.To study the effects of body composition,physical activity,nutritional status and sleep situations on muscle function in the elderly.3.To establish a risk predictive model of muscle functional impairment in the elderly to provide a basis for early screening and prevention.Methods:1.340 elderly people over 60 years old were selected used convenient sampling method in the township community of Guangling District,Yangzhou City.Then,we conduct the test of anthropometric indicators,body composition,and muscle tunction for the elderly,as well as a questionnaire of general information,physical activity,nutritional status,and quality of life,and analyze the relationship among muscle function and body composition,physical activity and nutritional status.2.The PSQI scale was used to assess the sleep status of the elderly.>7 points is diagnosed as sleep disorders,?7 points is diagnosecl as normal sleep,and the effects of sleep situations on muscle function were analyzed.3.Based on the existing diagnostic criteria of muscle functional impairment,the elderly in the community were divided into two groups.Logistic regression model was used to establish the risk prediction model of muscle functional impairment in the elderly,and the predictive value of the model was evaluated by ROC curve.Results:1.There were significant differences in muscle function,calf circumference,body composition related indicators and quality of life between male and female older people(P<0.05).With age,muscle function,BMI,calf circumference and upper arm circumference,body composition indicators(skeletal muscle mass,skeletal muscle mass index,fat-free mass,waist hip ratio,protein content,basal metabolism and the total energy expenditure),physical activity were statistically different between different age groups(P<0.05).2.There were significant differences in body composition indicators(skeletal muscle mass,skeletal muscle mass index,fat-free mass,protein content,basal metabolism and total energy expenditure)between the two groups of normal muscle function group and muscle functional impairment group in the community(P<0.05).In addition,physical activity(P=0.001),nutritional status(P=0.033),quality of life(P<0.001)were statistically significant between the two groups.Correlation analysis showed that muscle function indicators(handgrip strength and short physical performance battery)were all related to body composition,physical activity,nutritional status and quality of life(P<0.05).Multiple linear regression analysis showed that calf circumference,skeletal muscle mass,percentage of body fat,physical activity and nutritional status were the influencing factors affecting muscle function in the elderly.3.There were significant differences in muscle function indicators(handgrip strength and short physical performance battery)and quality ot life between the two groups of normal sleep and sleep disorder(P<0.05).Correlation analysis of muscle function(handgrip strength and short physical performance battery)and sleep showed that handgrip strength was negatively correlated with PSQI score(r=-0.132,P=0.016),and short physical performance battery was negatively correlated with PSQI score(r=-0.140,P=0.010);After establishing the logistic regression model by adjusting the age and gender,sleep disorders was still the influencing factor of muscle functional impairment(OR=2.006,95%CI:1.171?3.435,P=0.012).4.Univariate logistic regression analysis showed that age,widowed condition,chronic disease,calf circumference,skeletal muscle mass,skeletal muscle mass index,fat-free mass,physical activity,sleep disorder was statistically significant.After multivariate logistic regression analysis,the four factors were finally included in the regression model,which were age,calf circumference,skeletal muscle mass and sleep disorder(PSQI>7 points).The ROC curve was draw using the joint predictor.The area under the curve was 0.726,the corresponding sensitivity was 68.09%,and the specificity was 70.43%.Conclusions:1.There are significant differences in muscle function,calf circumference,body composition and quality of life between male and female older people.In addition,muscle function,body composition and physical activity changed with age.2.Muscle function is related to body composition,physical activity,nutritional status and quality of life.Body composition,physical activity,and nutritional status are factors influencing muscle fianctional impairment.3.Sleep disorders is related to quality of life and is also a contributing factor to muscle functional impairment.4.Age,calf circumference,skeletal muscle mass and sleep disorder(PSQI>7 points)are independent influencing factors of muscle mnctional impairment,and the combined predictive model has certain value for predicting the risk of muscle functional impairment.
