Objective:To study Comparison of clinical effect between modified hysterectomy and vaginal hysterectomy.Methods:56 cases of patients who underwent large hysterectomy due to uterine enlargement caused by benign diseases such as uterine fibroids and the size greater than 12 weeks of pregnancy from June 2015 to October 2018 were selected for the study,according to the surgical method,it was divided into two groups:one group was modified large uterus total laparoscopic resection(MTH group);the other group was modified large uterus total laparoscopic resection(MTH group).The other group was laparoscopic-assisted vaginal large hysterectomy(LAVH group).There were 16 cases in MTH group and 40 cases in LAVH group.The length of hospital stay,hospitalization cost,operation time,intraoperative blood loss,postoperative exhaust time,time to get out of bed,patient satisfaction and complications were compared between the two groups.Results:There was no significant difference in operation time between the two groups.The amount of intraoperative blood loss and length of stay in the MTH group were significantly lower than those in the control group(P<0.05).There was no significant difference in postoperative exhaust time between the two groups.The time of getting out of bed and the cost of hospitalization in the MTH group were significantly lower than those in the control group(P<0.05).The post-treatment satisfaction of the MTH group was 93.75%,which was significantly higher than that of the control group(65.00%).The satisfaction rate of the two groups was significantly higher(P<0.05).The total incidence of complications in the two groups was 0.00%and 22.50%,respectively,with significant difference(P<0.05).Conclusion:For the treatment of hysterectomy,MTH and LAVH are safe and effective.(2)Modified total laparoscopic hysterectomy has some advantages over laparoscopic assisted transvaginal hysterectomy in terms of hospitalization days and intraoperative bleeding,but there are no significant differences in operation time,postoperative exhaust time and hospitalization costs.Modified total laparoscopic hysterectomy for giant uterus is a safe and feasible surgical method with less damage to human body and quick recovery.The operation is worthy of clinical application and promotion.
