| ObjectiveThe purpose of this study was to study the common syndromes of dysphagia after apoplexy and the regularity of syndrome and treatment of traditional Chinese medicine(TCM)combined with acupuncture and medicine in the treatment of this disease,and to evaluate the efficacy and safety of TCM acupuncture and medicine in the treatment of this disease from the point of view of evidence-based treatment.Method1.Research on the law of syndrome and treatment based on data mining: the computer network is used to effectively retrieve the common literature databases such as Weipu Network,China knowledge Network,Wanfang Database,CBM and so on.The retrieval time is from the establishment of the database to 2019,which is included in the clinical research of all acupuncture and medicine in the treatment of this disease.SPSS 19.0 was used for frequency analysis,systematic cluster analysis and Clementine 12.0 for association rule analysis.2.Research on efficacy evaluation based on systematic evaluation: based on the above retrieval strategies,the retrieval time was searched in the above commonly used Chinese database and English database.According to the inclusion and exclusion criteria,all random and quasi-randomized controlled trials were included in the treatment of the disease with combination of acupuncture and medicine.Meta analysis was carried out by Revman5.3 software,and the relative risk was used as the combined effect.The evidence quality grade of the results was analyzed by GRADEpro3.6 software.Result1.Research on the Law of Syndrome and treatment based on data Mining(1)Syndrome research: there are 34 literatures that meet the inclusion and exclusion criteria,among which wind-phlegm blocking collaterals syndrome combined with acupuncture and medicine intervention is the most literature.(2)Study on the law of medicine points: the drugs with the highest frequency of use are drugs for tonifying deficiency and promoting blood circulation and removing blood stasis.The first four acupoints and drugs with the highest frequency were Lianquan,Fengchi,Jinjin,Yuye,Shichangpu,Gancao,Banxia and Yuanzhi,respectively.The main selection of meridian strange points and Ren pulse,gallbladder meridians,du pulse,stomach meridians on the acupoints.(3)The association degree of high-frequency medicine points:the combination of the highest degree of correlation is:Jinjin-Yuye,YujinShichangpu.Correlation rule analysis showed that the highest correlation between the point and the drug was that of Chuanxiong-Lianquan and Dannanxing-Lianquan.(4)Cluster analysis of high frequency drug points: six effective clustering groups were obtained.2.Research on curative effect Evaluation based on system EvaluationFinally,32 articles were included in the study.Meta analysis and GRADE evidence quality grade analysis showed that:(1)the total effective rate of TCM acupuncture combined therapy was better than that of simple acupuncture,simple traditional Chinese medicine,western medicine and blank group,the quality of evidence was medium,and the recommended intensity was weak.(2)the cure rate of TCM acupuncture combined with medicine was better than that of simple acupuncture and blank group,and the quality of evidence was medium.The recommended intensity is weak.(3)the cure rate of traditional Chinese medicine combined with acupuncture and medicine is better than that of traditional Chinese medicine and western medicine,the quality of evidence is very low,and the recommended intensity is very weak.(4)the incidence of pneumonia treated by traditional Chinese medicine combined with acupuncture and medicine is lower than that of simple traditional Chinese medicine and blank group,and the evidence is lower than that of simple traditional Chinese medicine and blank group.The quality is very low and the recommended intensity is very weak.(5)compared with traditional Chinese medicine in the treatment of this disease,the incidence of adverse reactions is different,and the difference is not statistically significant.Conclusion1.The meridian and veins passing through the neck and glossopharyngeal part are common meridian for the treatment of this disease,and the upper acupoints and meridian points are common acupoints,and the combination rules are mainly used to supplement the qi and blood and blood stasis used in combination.2.The bias risk map of systematic evaluation showed that the quality of literature included in this study was low.GRADE evidence quality grade analysis showed that the evidence quality grade of each group was medium or very low,and the recommended intensity was weak,which indicated that acupuncture and medicine combination was beneficial to the recovery of swallowing function after stroke,but the evidence was still insufficient due to the lack of high quality related literature,and a large number of high quality large sample random controlled studies were needed to confirm the curative effect. |
