| Objective : Analysis of the effects of three-dimensional titanium mesh cranioplasty(Skin flap free time,Bleeding volume during operation and Average operative time),postoperative complications and the effect on the recovery of nerve function after operation in the early(1-3 months) cranial repair of the skull defect after the decompression of bone flap.To explore whether early cranioplasty is superior to conventional surgery.Methods : Retrospective analysis was performed on 70 patients with craniocerebral defect after craniocerebral injury from January 2013 to December 2017.According to different stages of cranioplasty,decompressive craniectomy was divided into two groups: early group(1-3 months) and regular group(3-6 months).Compared two groups of patients’ average operation time,intraoperative blood loss,intraoperative flap separation time,6 months after corresponding complications of two groups of patients,and through the Glasgow score(Glasgow outcome scale,GOS),the National Health Institute’s neurological deficit score(NIHSS) and the long term Cartesian functional status(Karnofsky performance status,KPS) to compare two groups of patients after two weeks of decompression of skull flap of nerve function and the skull to repair the postoperative 1 month,3 months,6 months of neural function recovery.Results: The average time of operation in the early group was shorter,and the scalp peeling time was shorter than that in the conventional repair group(P<0.05),and the bleeding volume was significantly reduced(P<0.05).There was no significant difference in the incidence of complications between two groups after cranioplasty(P> 0.05).In the early group,the GOS score,NIHSS score and KPS score were not statistically significant compared with the conventional group(P>0.05)in the early group(P> 0.05);the early group was 3 months after the operation.The scores of GOS,NIHSS and KPS in 6 months were significantly higher than those in the conventional group(P<0.05).Conclusion:In the absence of obvious contraindications,cranioplasty can be carried out in the early 1-3 months after craniocerebral trauma decompression of craniocerebral trauma.It can reduce the free time of the flap,the amount of bleeding and the total operation time in th e operation.It is also beneficial to the recovery of the nerve function after the operation.It is worth popularizing and applying in the clinic. |
