Clinical Observation On Treatment Of Primary Dysmenorrhea Of Cold Coagulation And Blood Stasis Type With Filiform-fire Needle

Objective:To compare the efficacy of filiform-fire needle acupuncture and ibuprofen sustained-release capsule in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type.Methods:62 cases of primary dysmenorrhea of cold coagulation and blood stasis in department of Gynecology,acupuncture and moxibustion of Zhongshan Hospital affiliated to Guangzhou University of traditional Chinese Medicine were divided into two groups according to random number table:30 cases of filiform-fire needle group,32 cases of drug group.Two acupoints(Group A and group B)were selected by the filiform-fire needle group.The acupoints of the two groups were used interchangeably every other day.Group A took Sanyinjiao(SP6)(left),GuanYuan(CV4),Mingmen(GV4),Group B took middle pole(CV3),Sanyinjiao(SP6)(right),Shenshu(BL23)(double);The control group was treated with ibuprofen sustained-release capsule orally,0.3g/times,twice a day(or when in pain).Both groups began treatment 5 days before menstruation,took 5 days or stopped taking until pain relief,continued treatment for 3 months,followed up for 3 months.Record the treatment of two groups of patients Pain.visual analogue score(VAS),dysmenorrhea symptom score before and after treatment,and serum levels of prostaglandin F2,E2(PGF2α,PGE2)were detected.Results:There were 28 cases in the filiform-fire needle group and 31 cases in the drug group.(1)VAS score:At each time point,the VAS scores were significantly lower in the two groups(F=130.318,P=0.000<0.01),but the VAS scores of the filiform-fire needle group at the same time point were lower than those of the drug group(F=10.318,P=0.002<0.05).(2)Dysmenorrhea symptom score:At each time point,the Dysmenorrhea symptom scores were significantly lower in the two groups(F=32.749,P=0.000<0.01),but the scores of the filiform-fire needle treatment group at the same time point were lower than those of the drug group(F=4.549,P＝0.037<0.05).(3)The levels of PGF2α and PGE2:The level of serum PGF2α in the two groups was lower than before treatment,but there was no statistical significance in the decrease of drug group(P=0.123>0.05),and the decrease in the filiform-fire needle group was more obvious than that in the drug group(P=0.001<0.01).In the two groups,the level of serum PGE2 were higher than before treatment,but there was no statistical significance in the increase of drug group(P=0.635>0.05),and the increase in the filiform-fire needle group was significantly higher than that in the drug group(P=0.029<0.05).Conclusion:The effects of filiform-fire needle in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type were better than those of oral ibuprofen sustained-release capsule,the mechanism of which was related to PGF2α and PGE2.
