Periodontitis is a common cause of oral disease and a major cause of tooth loss in adults.It has a high prevalence in China.At the same time,periodontitis is also the most common chronic infectious lesion,which is associated with mucosal immunity and can cause systemic inflammatory reactions.In recent years,there are more and more studies committed to the interaction between periodontitis and other systemic diseases like coronary heart disease,diabetes,lung disease,premature birth,obesity and other diseases,etc.Chronic kidney disease is a general term for a type of disease,which refers to kidney damage caused by different causes for more than three months,or glomerular filtration rate of less than 60ml/min/m2 for more than three months.Chronic periodontitis and chronic kidney disease have many common risk factors,such as diabetes,hypertension,hyperlipidemia,infection and inflammation,age,smoking,etc.,which attracts lots of scholars to study the correlation between these two diseases.IgA nephropathy is currently the most common primary glomerular disease in the world.Now studies have shown that there is a close relationship between periodontitis and chronic kidney disease.Most researches are epidemiological studies and clinical descriptions.The strength of the argument is weak.The causal link and the exact mechanism between chronic periodontitis and IgA nephropathy remains to be confirmed.Objective:By establishing a compound model of chronic periodontitis and Immunoglobulin A nephropathy in rats,the relationship between chronic periodontitis and immunoglobulin A nephropathy is preliminarily studied byusing periodontal indicators,renal pathological staining andserum CRP and IL-6 levels.Methods:Eighty Sprague-Dawley rats were selected.The periodontitis model was established by orthodontic wire ligation and periodontal pathogen infection.The IgA nephropathy model was established by BSA gavage,subcutaneous injection of CCl4,and tail vein injection of LPS.The periodontal index,renal clinical and pathological parameters,and serum CRP and IL-6 levelsof the rats were detected and analyzed statistically at the end of 12 weeks.Results:On the basis of successful modeling,the periodontal clinical indexes of CP+IgAN group,such as sulcus bleeding index,depth of probing and attachment loss,were significantly different from those of CP group,P<0.001;Renal histopathology and immunofluorescence scores in the CP+IgAN group were also more severe than in the IgAN group,P<0.001.At the same time,some renal tissue inflammatory changes occurred in the renal pathology of CP group,and the immunofluorescence staining score was higher than that of the control group,P<0.05.The sulcus bleeding index,probing depth and attachment loss of the IgAN group were higher than those of the control group.Some rats have gingivitis and even mild periodontitis.By measuring the levels of serum CRP and IL-6,the serum CRP levels of rats in CP group,IgAN group and CP+IgAN group were significantly higher than those in normal control group,P<0.05.The average serum CRP level in CP+IgAN group was higher than that in the IgAN group(P<0.05).The IL-6 level in the CP group and the CP+IgAN group was higher than that in the normal control group and the IgAN group,P<0.05,and the IL-6 level in the CP+IgAN group was higher than that in the CP group.Significantly increased,P<0.05.Conclusion:There is a certain relationship between chronic periodontitis and IgA nephropathy,which may affect the development of each other.Periodontitis-IL-6-CRP-nephropathy may be one of the effective ways to influence the development of periodontal disease and nephropathy...
