Posterior Chamber Phakic Intraocular Lens(V4c)Implantation For High Myopia Of Retinitis Pigmentosa Patients:Early Study

Purpose:To assess the early performance of implantation of a posterior chamber phakic intraocular lens(pIOL;implantable collamer lens[ICL])with a central artificial hole(V4c;STAAR Surgical)for 2 high myopia patients with retinitis pigmentosa(RP).Design:Early(3 months)retrospective follow-up study.Methods:Evaluated 4 eyes of 2 RP patients with high myopia who underwent ICL(V4c)implantation,and one of them underwent torie ICL(V4c)implantation.We assessed the diopter,safety and efficacy of surgery,intraocular pressure(IOP),endothelium to the anterior surface of the pIOL(E-pIOL),central vault,corneal endothelium density and visual field after 1 month and 3 months.The patients eompleted a questionnaire detailing symptoms on a quantitative grading scale(refer to National Eye Institute Refractive Error Quality of Life Instrument-42[NEI RQL-42])after 3 months.We compared the preoperative and postoperative scores of scales to evaluate the patient subjective symptoms.60 is the base line and there are five levels,a higher score implies a higher self-reported quality of life.Results:The preoperative changes of visual acuity,diopter and visual field were stable in 2 RP patients with high myopia.After being followed up for 3 months,postoperative mean logMAR uncorrected distance visual acuity and mean logMAR corrected distance visual acuity were 0.1,0.3,0.4,0.4 and 0.1,0.2,0.2,0.2,respectively,at 3 months after surgery.The safety and the efficacy of surgery were 2.00,1.50,3.50,2.50 and 2.00,1.00,1.75,1.25,respectively,at 3 months after surgery.4 eyes were within ± 1.0 D of the targeted correction.No significant rise in IOP and on vision-threatening complications associated with surgery occurred during the observation period.The post-operative questionnaire showed that except for glare,the postoperative scores of scales(refer to[NEI RQL-42])were almost improved or flat compared with those before operation.Especially the scores of visual clarity,far vision,diurmal fluctuation(night visual acuity)and activity limitations from "very poor or poor" levels before surgery to "excellent or good" grades after surgery.And the postoperative glare scores were more than 85,also belong to "good" grade.Conclusion:According to our early study,ICL(V4c)implantation may provide predictable and stable refractive results in the correction of high myopia of RP patients,with good safety,effectiveness and satisfaction.Long-term stability and safety need further follow-up observation.
